About this file

This document contains a past performance questionnaire for contractors bidding on a solicitation to construct a new shop building at the DC Booth National Fish Hatchery. The solicitation seeks the construction of a single-story pre-engineered metal building containing wood and paint storage areas, a vehicle bay, and restroom. The new building must blend with the historic site aesthetically. The questionnaire evaluates contractors on factors such as management and personnel quality, quality control, problem solving, compliance, responsiveness, submittal timeliness, and resolution of punchlist items. Respondents are asked to rate performance as satisfactory or unsatisfactory and provide narratives to support their ratings. The completed questionnaires will be used to evaluate contractors' past performance for the solicitation. The contracting officer's point of contact is provided for any questions.
