SUBJECT: Request for Past Performance Evaluation

1. You have been identified as a point of contact for a past performance evaluation of the firm identified on the attached survey. This firm is currently interested in providing the Parade Uniforms for the Cadets at the United States Air Force Academy, CO.
2. Please complete the attached survey questionnaire. According to the rating scale, full successful performance is "Satisfactory". It is important to provide a narrative to support ratings other than "Satisfactory." Describing how the contractor exceeded or failed to meet standards will allow our evaluation team to validate the ratings against the definitions provided below. Unsupported ratings may be treated as "Satisfactory."
3. Your contribution and prompt attention to this questionnaire is greatly appreciated. Please email the survey to danielle.hyde@us.af.mil and jonathan.tringali.1@us.af.mil.
