About this file
This is a Past Performance Questionnaire issued by the 628th Contracting Squadron at Joint Base Charleston, South Carolina for solicitation FA441826Q0010, dated 5 March 2026. The questionnaire is designed to collect references and performance feedback from past customers of vendors bidding on a one-time service contract for temporary event rentals supporting the 2026 Joint Base Charleston Air Show. The contractor selected will be responsible for complete delivery, professional setup, and timely teardown of tents, tables, and chairs on the installation.
The questionnaire requests respondents to complete sections documenting respondent information, vendor reference information, contract information including performance period and contract value, and past performance assessments across four key evaluation areas: ability to meet work requirements and quality standards, on-time and within-budget order completion, responsiveness to quality issues and deficiencies, and communication responsiveness with the Government during and after contract performance. Respondents rate performance using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) and must provide explanations for any ratings other than Satisfactory, including supporting documentation such as performance evaluations or discrepancy reports. The questionnaire also requests overall performance ratings, determination of contract terminations, and whether the firm would be awarded future work. Completed questionnaires must be returned by 4:00 PM EST on 5 March 2025 to Contract Specialist Leo Lachapelle at leo.lachapelle.2@us.af.mil and Matthew Michel at matthew.michel@us.af.mil.
