About this file

This is a Department of Interior Past Performance Questionnaire (PPQ) template used to evaluate contractor performance on past projects relevant to the Little Creek Campground Design-Build solicitation (140L0626R0005). The questionnaire is a two-part document where offerors first complete Section A with project details including contract information, location, company identification, and key dates and amounts, then submit it to the project owner or evaluator for completion of Section B.

Section B requires the evaluator to assess the contractor's performance across five primary evaluation areas using a six-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, and Not Applicable). The evaluation areas are: Quality (workmanship, quality control adherence, and technical submittals); Schedule and Time Management (milestone compliance, responsiveness to requests, and timely issue resolution); Pricing and Payments (cost management, modification pricing fairness, invoice accuracy, and supplier payments); Management (project oversight, subcontractor coordination, safety, remote location capabilities, and problem-solving); and Regulatory Compliance (building codes, life safety codes, and labor standards). Completed questionnaires must be submitted by the deadline specified in the solicitation and cannot be accepted late. Each offeror may submit only one past performance reference per project, and all referenced projects must have been completed within five years preceding the solicitation date. The evaluator must sign the completed questionnaire for it to be considered valid.
