About this file
This document provides details for a draft request for proposal for athletic trainer services. The U.S. Army Medical Command is contemplating awarding a personal services contract to provide qualified and certified athletic trainers to designated units at the brigade, battalion, or company level. Services may include providing musculoskeletal care, early medical intervention, injury prevention, and performance optimization. Athletic trainers would be assigned to fitness centers, unit areas, outdoor and field environments, and soldier performance centers. Any medical treatment would be subject to supervision by a government healthcare provider. EXHIBIT F
CLINICAL COMPETENCY ASSESSMENT TOOL (CCAT)
CRYOTHERAPY
TASK GO NO GO COMMENTS
1. Gather all materials needed to perform a cryotherapy treatment.
2. Identify the area to be treated and explain procedure. Explain the sensations of cold, burning, aching, and numbness.
3. Position the patient, expose the area to be treated, and appropriately drape the patient as needed.
4. Apply cryotherapy to patient and set timer as appropriate:
a. Ice Pack – 10 to15 minutes *ensure use of barrier between ice and skin
b. Ice Massage – 8 to 10 minutes
c. Ice Bath – 8 to 10 minutes
5. Discontinue treatment after the timer has gone off. Inspect the area for adverse reactions. Monitor patient status following treatment.
6. Describe appropriate patient management process if adverse reaction is noted.
7. Demonstrate good understanding of indications for use:
a. Prevent further swelling
b. Reduce muscle spasm and/or pain
c. Reduce inflammation
8. Demonstrate good understanding of contraindications for use:
a. Circulation or sensory problems
b. Hypersensitivity/allergy to cold
c. Raynaud’s or other vasospastic diseases

ELECTROTHERAPY
1. Inspect equipment to ensure it is in good working order and gather all materials needed to perform electrotherapy treatment.
explain procedure. Explain that they may feel a tingling sensation and/or muscular contraction in the area.
4. Select the electrotherapy method most appropriate for the condition to be treated.
a. TENS – Pain management
b. IFC – Pain management, increase circulation, and decrease muscle spasm
c. HVGS – Pain control, edema reduction, tissue healing, and decrease muscle spasm
d. d) NMES – Muscle re-education
5. Turn the unit on and set parameters for type, frequency, and duration as appropriate to the patient’s condition.
6. Clean the treatment area of skin oils or lotion and apply electrodes as indicated. Secure electrodes to the area with tape or elastic straps.
Press START, then gradually increase intensity to desired level.
7. Once treatment is concluded, remove electrodes from the skin, clean off any coupling medium, and turn off the equipment. Inspect the area for adverse reactions. Monitor
8. Describe appropriate patient
9. Demonstrate good understanding of
a. Reduce muscle spasm and/or
b. Improve muscular contraction
d. d. Increase circulation
10. Demonstrate good understanding of contraindications for use:
a. Circulation or sensory problems
b. Pregnant uterus or cardiac pacemaker
c. Cancerous lesions or active infection
d. Significant muscular injury
(NMES)

THERMOTHERAPY (Hot Packs)
1. Gather all materials needed to perform thermotherapy treatment.
Ensure hydrocollator temperature is maintained at 165-170° F explain procedure to the patient.
4. Apply hot pack to the patient with the appropriate toweling (6-8 layers) and secure as needed. Set timer (15-20 minutes) and provide patient with a bell to alert you if the pack gets too hot.
5. Discontinue treatment after the timer has gone off. Inspect the area for adverse reactions. Monitor
6. Describe appropriate patient
7. Demonstrate good understanding of
a. Pre-heat prior to activity
b. Reduce muscle spasm and/or
c. Increase extensibility of soft tissues
8. Demonstrate good understanding of contraindications for use:
a. Circulation or sensory problems
b. Acute inflammatory conditions
c. Infection and/or fever

ULTRASOUND
1. Inspect equipment to ensure it is in good working order and gather all materials needed to perform ultrasound treatment.
explain procedure. Explain that they may feel a warming sensation but that a burning or aching sensation is not normal and that the patient should indicate immediately if they feel either of these.
4. Select the appropriate size sound head for the area to be treated and apply coupling medium to the area or the sound head. Turn the unit on and set parameters for frequency and duration as appropriate to the condition the patient is being treated for:
a. 1MHz for deep structures, 3MHz for superficial structures or bony prominences
b. Pulsed for acoustic streaming effect, continuous for thermal effect
c. Duration of treatment is 8 to 10 minutes
5. Apply sound head to the area to be treated. Keeping sound head in contact with the treatment area, press START, then gradually increase intensity to desired level as you move the sound head (typically ≤ 1.0 W/cm2 for acute conditions,
1.0 to 2.0 W/cm2 for chronic conditions).
6. Maintain even contact with the skin, keep the sound head constantly moving (4cm/sec), and avoid bony prominences if possible throughout the treatment. Do not lift the sound head off the patient for any more than a few seconds.
7. Once treatment is concluded, remove sound head from the skin and turn off the equipment. Inspect the area for adverse reactions.
Monitor patient status following treatment.
8. Describe appropriate patient
9. Demonstrate good understanding of
a. Increase collagen elasticity
b. Reduce muscle spasm and/or pain
d. Promote tissue repair
10. Demonstrate good understanding of contraindications for use:
a. Circulation or sensory problems
b. Near pregnant uterus
c. Cancerous lesions or active infection
d. d. Epiphyses in adolescents/children
