About this file
This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on August 11, 2017. The form serves as an informational guide for potential bidders on construction projects and outlines common problems that occur during the bidding process. The document specifies that all bidders must thoroughly review the complete request for quotation documents, including all addenda, instructions, and supporting materials, as failure to comply with stated requirements may result in bid disqualification. The form does not establish specific project details, quantities, timelines, or award dates, but rather functions as a general checklist to ensure bidder compliance across multiple construction projects.
The form identifies two categories of disqualification errors: those resulting in immediate bid rejection and those that may disqualify bids before contract award. Immediate disqualification grounds include failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to submit valid bid bonds, non-compliance with mandatory RFQ requirements, failure to acknowledge addenda when required, late bid submission, federal or state debarment or suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints. Additional requirements that must be resolved prior to contract award include resolution of any debt to the state or political subdivision, Workers' Compensation or Unemployment Compensation delinquency, proof of required bonds and insurance, submission of a valid West Virginia contractor's license number, submission of a signed drug-free workplace affidavit, and provision of a complete subcontractor listing. The document contains no specific pricing, set-asides, funding information, or contract term details.
