About this file
This document is a Request for Quote (RFQ) issued by the Illinois Department of Public Health (IDPH) for Archival Quality Certificate Binders. The RFQ specifies the required product details, including the binder style, size, materials, and features. Vendors must submit quotes by the bid opening date and time indicated in BidBuy. The contract will be a one-time purchase, and the winning vendor must possess all mandatory qualifications and offer the lowest bid. Pursuant to State of Illinois regulations, vendors registered in the Small Business Set-Aside Program may subcontract up to 49% of the contract value to non-registered businesses. The RFQ also includes COVID-19 protection requirements, such as face coverings, vaccinations, or testing, that the vendor must adhere to as applied by the Agency.
The RFQ outlines pricing terms, noting that quotes must be valid for 90 days and include all expenses. If the resulting contract value exceeds $50,000, financial disclosures are required. Vendors must complete and submit the appropriate financial disclosure forms based on their registration status in the Illinois Procurement Gateway. The Agency reserves the right to reject all offers or individual offers that fail to meet any requirement.
