About this file
This is a Required Certifications document (Appendix I) for an Invitation to Bid (ITB 2526 074) issued by the State of Florida, Department of Children and Families (DCF), for Wastewater Surveillance services. Respondents must provide organization official business information, including name, address, and Federal Employer Identification Number, along with primary and alternate contact persons who possess authority to bind the organization to a contract. The document establishes mandatory certifications that respondents must acknowledge, including acceptance of solicitation terms and conditions, confirmation of no conflicts of interest, absence of disqualifying involvement, no collusion in the bidding process, prohibition of gratuities, and verification that the organization is not listed on various state and federal exclusion lists such as the Convicted Vendors List, Discriminatory Vendors List, Suspended Vendors List, Antitrust Violator Vendor List, and Scrutinized Companies Lists.
The document also provides optional tiebreaking certifications that qualifying respondents may claim to gain competitive advantage if bids are equal in price, quality, and service. These tiebreakers include certification as a Minority Business Enterprise, Veteran Business Enterprise, Florida Business, Foreign Manufacturer with a Factory in Florida, or Drug-Free Workplace, in accordance with applicable Florida Statutes. Respondents must indicate which tiebreaking certifications apply and must submit all required certifications and signatures of both primary and alternate contact persons on or before the Response Due Date to maintain eligibility for consideration under the tiebreaker provisions.
