About this file
This is a Bid Opportunity List form (Appendix H) issued by the Wisconsin Department of Transportation in compliance with 49 Code of Federal Regulations Part 26.11 for the Shared Ride Taxi Service contract opportunity for the City of Viroqua and City of Westby. The form is designed to document all firms participating or attempting to participate on this DOT-assisted contract, including prime contractors, subcontractors, subconsultants, and suppliers. The list must capture participation across all bidding tiers, regardless of Disadvantaged Business Enterprise (DBE) status. For consulting companies, the form requires documentation of all subconsultants expressing interest in teaming on the project. The contract term spans two years from January 1, 2023, through December 31, 2024, with three additional one-year renewal options available for a potential total of five years.
Prime contractors and consultants must complete all columns of the form, providing bidder name, contact information, DBE status, firm age, type of work, and annual gross receipts categorized into ranges from less than $500,000 to $5 million and above. Subcontractors and subconsultants require name and contact information to be completed by prime bidders, with additional information on DBE status, firm age, work type, and gross receipts provided as available. The form establishes a standardized mechanism for tracking bidder participation and financial capacity across the shared ride taxi service procurement, which provides door-to-door service seven days a week available to the general public in both jurisdictions.
