About this file
This is an Apprenticeship Certification form from the State of Colorado, Office of the State Architect, State Buildings Program, applicable to public construction projects valued at $1 million or more. The certification requires general contractors to document that all identified firms for mechanical, sheet metal (MEP work only), fire suppression, sprinkler fitting, plumbing, and electrical trades participate in apprenticeship programs registered with the United States Department of Labor's Employment and Training Administration or state apprenticeship councils recognized by the DOL. Contractors must provide supporting documentation verifying that each firm has maintained a proven record of graduating a minimum of twenty percent of its apprentices for at least three of the past five years. The certification statement references Colorado Revised Statute 24-92-115 (SB 19-196) and requires the general contractor to execute the certification by providing the date, month, and year of agreement.
The contracting agency is required to make all submitted documentation publicly available through its website within thirty days of receipt. General contractors must agree to provide additional documentation regarding affected apprenticeship training programs upon request. The form includes enforcement provisions stating that if a contracting agency determines a subcontractor has willfully falsified documentation or misrepresented qualifications, the general contractor must immediately terminate the subcontractor's contract and remove them from the project. The State may pursue debarment of the general contractor pursuant to C.R.S. 24-109-105 and pursue any other remedies available under law.
