About this file
This is a Cost Proposal template for the Rhode Island Department of Health's Prescription Drug Monitoring Program (PDMP) RFP 27007077. The template requires vendors to submit detailed cost proposals covering all scope and requirements across a base contract period from January 1, 2027 through April 30, 2028, with four optional 12-month renewal periods extending through April 30, 2032. Vendors must itemize costs for design, development, implementation, and support/maintenance phases, organized by payment milestones and fiscal periods. The template includes sections for Services, Software, Hardware, Consulting Retainer, Technical Support, Data Updates, Reporting Support, Data Source Maintenance, and Vendor Hosting costs. Additionally, vendors must submit a Proposed Team and Rate Cards section identifying all key staff positions, their hourly rates, project duration, and hours reserved for consulting retainer services across each contract period.
Pricing terms are firm and fixed unless otherwise negotiated during final contract discussions, with all travel and miscellaneous expenses required to be included in service costs. Vendors must clearly identify any fixed costs and specify when two or more costs are bundled into a single fee. The State will withhold a 0.33 percent contract administrative fee from all invoices, and payment terms will be finalized during contract negotiation. All completed Excel worksheets must be submitted with authorized signatures and include a statement confirming the vendor's agreement to all RFP terms, conditions, and specifications. The evaluation process weights technical merit, cost, and Minority Business Enterprise (MBE), Women Business Enterprise (WBE), and Disability Business Enterprise (DisBE) participation rates, with the lowest-cost proposal receiving 100 percent of available cost points.
