About this file
This is a Vendor Disclosure Statement issued by the State of Colorado on July 19, 2024, pursuant to Colorado Revised Statute 24-102-206, applicable to the Thermal Opportunity Mapping contract opportunity. The form is required for all state contracts and solicitations for services executed after August 3, 2007, and must be completed by vendors to the extent that contract performance information has not been disclosed in the Statement of Work. The disclosure statement specifically addresses contract performance conducted outside the United States or the State of Colorado, requiring vendors to identify the geographic location(s) where services will be performed, explain the necessity or advantage of performing work outside Colorado or the United States, and provide the contract routing number and vendor name.
The form clarifies that completion is not required for contracts to which the State is a party under specific programs, including Medicare, the Colorado Medical Assistance Act (Articles 4 to 6 of Title 25.5 CRS), the Children's Basic Health Plan Act (Article 8 of Title 25.5 CRS), or the Colorado Indigent Care Program (Part I of Article 4 of Title 25.5 CRS). Vendors must answer whether any services or subcontracts will be performed outside the United States or Colorado, with affirmative responses triggering completion of the three supplementary questions regarding work location, justification, and contract identification.
