About this file

This is a Vendor Disclosure Statement form issued by the State of Colorado on July 19, 2024, applicable to all state contracts and solicitations for services executed after August 3, 2007. The form requires vendors to disclose whether any services under a contract or subcontracts will be performed outside the United States or Colorado, as mandated by Colorado Revised Statute 24-102-206. If services are to be performed outside these jurisdictions, vendors must identify the specific countries and states where work will occur and provide justification for performing services outside the United States or Colorado. The form requests the contract routing number and vendor name as part of the disclosure process.

The form explicitly exempts certain contracts from the disclosure requirements, including those related to Medicare, the Colorado Medical Assistance Act, the Children's Basic Health Plan Act, and the Colorado Indigent Care Program. This disclosure mechanism serves as a compliance tool for the State of Colorado to monitor contract performance locations and ensure transparency regarding any offshore or out-of-state service delivery arrangements prior to contract execution.
