About this file
The document is a Minimum Qualifications Checklist from San Mateo County, California, specifically for the Human Services Agency's (HSA) Core Service Agencies - Center on Homelessness procurement. The county is seeking proposals from qualified agencies to provide emergency and safety net services as part of the County's Core Service Network, with services including emergency food assistance, housing information and support, homelessness prevention referrals, Coordinated Entry System access, and other immediate basic needs support.
Proposing agencies must meet several key qualification requirements, including being legally authorized to conduct business in California, not being on federal debarment lists, having at least three years of experience providing safety net services in San Mateo County, and being registered in the System and Award Management (SAM) database. Agencies must complete this checklist, providing details such as their registered business name, Unique Entity ID, and certifying the accuracy of their submission. The document indicates that proposals will be evaluated based on these minimum qualifications, with agencies required to demonstrate their capability and compliance before being considered for the contract.

5. Please confirm your firm is registered with System and Award Management (SAM). SAM.gov* Comment by Jessica Silverberg: P&C- is this correct? It seems like there are 2 questions meshed into 1 here- agreement to county terms and conditions, and SAM.gov. seems like this should be 2 separate questions. Comment by Jessica Silverberg: P&C- also- the question on the terms and conditions is not worded that clearly. I think in past versions, the response options were clearer- something like- agree to terms and conditions, or we are proposing the attached changes. Please revise to be clearer- if departments are allowed to make changes to this section Comment by Sherman Gee: This is from the central procurement RFP template and they change it frequently. We can edit to separate into two questions and clarify Comment by Chris Ratana: P&C - I take this as OK to leave? Comment by Khalia Parish: Edited per Jessica’s suggestion.
