Appendix A - Evaluation Forms Summary
This document comprises evaluation forms for the State of Colorado Office of the State Architect State Buildings Program, designed to assess architectural, engineering, and consulting services firms for the Commissioning Service for Longmont Urban Campus project. The evaluation process consists of three components: a prequalification submittal evaluation form, an oral interview evaluation form, and a qualification-based selection ranking matrix. The prequalification evaluation assesses firms across six primary categories: project team qualifications and experience, firm and team capabilities, prior relevant experience, project approach including budget and quality control methodologies, equity and diversity initiatives, and work location familiarity. Each criterion receives a weighted score multiplied by a numerical rating of 0 to 5, with 0 indicating missing information, 1 representing unacceptable qualifications, and 5 indicating excellent performance. The oral interview evaluation form follows a similar weighted scoring structure across five categories: project team, team capabilities, prior experience, project approach, and work location, with evaluators assigning ratings of 0 to 5.
The ranking matrix consolidates evaluation results from multiple evaluators to determine the most qualified firm for selection. Agencies and institutions are encouraged to customize evaluation criteria to reflect unique project characteristics and must establish weights using whole numbers prior to evaluation. The prequalification submittal must include minimum requirements and a letter from a surety, along with acknowledgment and attestation documentation. Evaluators assess the strength of each firm's qualifications based on key staff involvement, project management capabilities, subconsultant experience, quality control methodologies, schedule adherence, and demonstrated success on similar projects. The optional passing score threshold, expressed as a percentage of total available points, should be predetermined by the agency institution before evaluations commence.
