About this file
This is an appendix to a prospectus for campground and related Granger-Thye concessions at the Chequamegon-Nicolet National Forest operated by the United States Department of Agriculture. The appendix establishes water testing requirements and septic system locations for the Mondeaux water system, which is classified as a public well and Transient Non-Community System (TN). The permit holder must conduct two types of testing at state-certified laboratories: annual nitrate sampling from the faucet after the pressure tank (with Forest Service notification required if results exceed 5 mg/l), and coliform sampling at specified frequencies. Year-round systems require one routine coliform sample per calendar quarter, while seasonally operated systems require a preseason sample prior to public service and routine samples for each month of operation. Coliform samples must be collected from four locations: men's restroom sink, dish sink, vegetable sink, and a sample faucet prior to the pressure tank. Positive total coliform results require permit holder notification to the Forest Service within 24 hours and DNR contact for further instructions; E. coli-positive results mandate immediate system shutdown and notification to both the Forest Service and DNR. The permit holder bears responsibility for repeat testing, paperwork, public notification, and corrective actions until compliance is achieved.
The appendix provides water testing resources including Forest Service regulations, system-specific information, state-certified laboratory contacts, Wisconsin DNR guidance documents, and the local DNR contact (Diane Ten Pas, Public Water Supply Specialist, Wisconsin Department of Natural Resources, Antigo, Wisconsin, contact information provided). The septic system location for the Main Lodge is identified as a concrete riser and lid visible above grade positioned between the storeroom and the path to the dam.

Appendix 20 Water Testing Requirements and Septic Locations

Water Testing Requirements, Resources and Septic Locations
A. Water Testing Requirements.
The following are water sampling and reporting procedures to be followed by the Permit Holder and Forest Service. The objective for developing these procedures is to ensure that our recreation water systems are safe for public use and to ensure that testing and reporting procedures are conducted in a timely manner according to State and EPA mandated procedures.
The Mondeaux water system is a public well and a Transient Non-Community System (TN).
Two types of tests are required: Nitrate and Microbiological (Coliform). Both must be completed at a state-certified lab. The permit holder must notify the Forest Service of results for each test. The sampling requirements are as follows:
1. Nitrate
a. One nitrate sample shall be collected annually. The sample location is as follows:
i. Sample faucet after pressure tank
b. If test result is >5 mg/1, contact the Forest Service permit administrator.
2. Coliform
a. The number of coliform samples required is determined by system usage.
i. If the system is operated year-round, one routine sample shall be collected each calendar quarter.
ii. If the system is operated seasonally, a preseason sample must be collected prior to serving water to the public, and routine samples shall be collected each full or partial calendar month the system is in operation. The preseason sample does not count toward the required routine sample for the month in which the system is opened.
• If any part of the system is depressurized, the system will be considered seasonal.
• Prior to re-opening to the public, the permit holder will need to follow the DNR Seasonal System Start-up Procedure and complete the Forest Service Annual Condition Survey
b. If a routine sample result is total coliform-positive, the permit holder shall contact the Forest Service within 24 hours of notification and contact the DNR for further instructions.
c. If a routine sample result is E. coli-positive, the permit holder shall shut down the system immediately upon notification from the lab and contact the Forest Service and DNR for further instructions
d. Repeat tests due to positive results are the permit holder’s responsibility. The permit holder will work with the DNR on all required paperwork, public notification, and corrective actions until the system is in compliance.

e. Coliform sampling locations are as follows:
i. Men’s restroom sink
ii. Dish sink
iii. Vegetable sink
iv. Sample faucet prior to pressure tank for Triggered, Raw Water, or Well sample – DNR will notify you if you need to take this sample (usually after a bacteria detect in one of the routine bacteria samples)
