About this file
This is Appendix 07: Standard Performance Evaluation Form for the United States Department of Agriculture Chequamegon-Nicolet National Forest, used to assess the performance of campground and related concessionaires operating under Granger-Thye permits.
The form evaluates permit holders across five major performance categories: Permit Terms, Operation & Maintenance Plan, Public Service, Meaningful Measures, and overall facility condition. Within these categories, evaluators assess 46 distinct performance items using a four-point rating scale (Above Expectations, Meets Expectations, Below Expectations, and Unacceptable). Critical elements, designated with an exclamation mark, include insurance requirements, timely payments, Title VI compliance, employee qualifications and training, public relations maintenance, fee and service delivery accuracy, safety inspections, hazard tree management, accessibility guidelines compliance, and facility functionality. The Meaningful Measures section specifically addresses health and cleanliness (human waste exposure, water and sewage systems, garbage management, litter removal, graffiti removal, and grease disposal), setting conditions (ecosystem protection, vegetation management, soil compaction prevention), safety and security (inspections, hazard correction, 36 CFR 261.14 compliance), responsiveness (accessibility, bulletin boards, staff training, visitor feedback mechanisms), and facility condition (functionality, toilet standards, maintenance, vandalism response, and road maintenance). The form includes spaces for comments, documentation of corrective actions, and signatures from both permit holder representatives and Forest Service representatives. Overall ratings follow specific rules: any Critical Element rated Below Expectations results in a maximum overall rating of Below Expectations; any Critical Element rated Unacceptable results in an Unacceptable overall rating; and three or more Critical Elements rated Below Expectations or more than four other elements rated Unacceptable results in an Unacceptable overall rating. Below Expectations ratings trigger a notice period for improvement before downgrade to Unacceptable, while Unacceptable ratings initiate the formal corrective action process that may lead to permit suspension or revocation.
