About this file
This is an Annual Required Affidavit for Bidders, Offerors, and Contractors for the Commonwealth of Kentucky, specifically related to the 62172-DAIL Senior Hunger Summit contract opportunity. The affidavit serves as a mandatory certification document that all bidders must complete and submit as part of their bid or proposal process. The document is effective for a maximum length of one year and requires the affiant to swear under penalty of perjury to compliance with multiple Kentucky state statutes and regulations. Key certifications include confirmation that neither the bidder nor any represented entity has violated campaign finance laws under KRS 45A.110 and KRS 45A.115, that all subcontractors have properly disclosed information required by KRS 45A.485, and that the bidding entity is properly registered with the Kentucky Department of Revenue for sales and use tax collection. Additionally, bidders must affirm they are not delinquent on state taxes or fees, are not engaged in prohibited boycotts as defined in Executive Order No. 2018-905, and have not violated prohibitions set forth in KRS 11A.236 during the previous ten years.
For non-bid contracts specifically, including sole-source, non-feasible-to-bid, or emergency contracts, additional certifications are required regarding campaign contributions limits established under KRS 121.056, KRS 121.330(1) and (2), and KRS 121.330(3) and (4). These provisions restrict contributions from the contractor, immediate family members with interests of 10% or more in the business entity, officers, employees, and affiliated entities to candidates with jurisdiction over the contract award. The affidavit must be completed by a duly authorized representative of the bidder, offeror, or contractor and notarized before submission. Failure to accurately disclose information may result in contract termination, repayment of funds, and other available legal remedies.
