About this file
This is an Annual Paperwork Packet for contractors seeking to perform work with the City of Toledo, Ohio in 2026. The packet comprises mandatory certification and compliance documents that all bidders must submit to be considered for contract awards. Key documents include Equal Employment Opportunity Certification affirming non-discrimination in hiring and employment practices, a Living Wage Affidavit certifying compliance with Toledo Municipal Code Chapter 187 requirements, and a Certification Regarding Debarment and Suspension confirming the contractor's eligibility to perform public work. The packet applies to the 2026 GCCS Construction Quality Assurance (CQA) project and other City contracts advertised beginning March 13, 2026. Contractors must submit these documents electronically through the City of Toledo's PlanetBids website along with a Letter of Transmittal, Line Items, Project Paperwork, City Professional Services Agreement, and three project references.
The Living Wage requirements mandate that employees receive a minimum hourly rate of $17.45 per hour or 110 percent of the updated federal poverty level for a family of four, whichever is greater, with optional health insurance coverage. Alternatively, contractors can pay a higher wage of $20.63 or 130 percent of the federal poverty level if health benefits are not provided. Eight exemptions are available for contractors meeting specific criteria, including those employing fewer than 25 persons, nonprofits, prevailing wage recipients, and Community Development Block Grant Fund recipients. Contractors must certify compliance with all applicable labor standards, post required notices to employees, provide notice to labor unions, and ensure subcontractors comply with the same requirements. The certification documents carry significant legal consequences, including potential contract cancellation, declaration of ineligibility for future government contracts, and criminal penalties of up to $10,000 in fines or five years imprisonment for false statements.
