About this file
This document is a Required Affidavit for Bidders, Offerors, and Contractors issued by the Commonwealth of Kentucky, specifically addressing compliance with state campaign finance laws under Kentucky Revised Statutes (KRS) 45A.110 and 45A.115. The affidavit is designed to be completed by a duly authorized representative of a contractor, bidder, or offeror, and is valid for one year from the date of execution. The form requires the representative to swear and affirm that the organization has not knowingly violated campaign finance laws and that the award of a contract will not violate any provisions of such laws.
The document outlines a mandatory attestation process where the contractor must complete the form, have it notarized, and submit it to the Commonwealth to be eligible for contract award or contract renewal. The affidavit includes spaces for the bidder or offeror's name, address, and Kentucky Vendor Code, as well as notary details. The form emphasizes that "knowingly" means the bidder or offeror is aware or should have been aware of any potential violations, and the Commonwealth retains the right to request an updated affidavit at any time.
