Contractor Name: ________________________________ DUNS#: ____________________ Address:________________________________________ Tax ID #: ____________________ Phone #:__________________ Fax #:_____________________ E- Mail: ___________________________

Name of Site Visit Representative: _______________________________

C.4.3 To the greatest extent possible, adverse slopes, rocky areas and areas supporting larger diameter trees have been identified on the ground. There may be other areas not yet identified that may not be able to be treated. If found, the contractor will notify the COR or Inspector as to location and reasoning for not treating the area. The COR will make the final judgment regarding the validity of the request and advise the CO of that decision.

C.4.4 Access into the Units will be from a closed trail beginning from the trail barrier. Re-establishment of the closure barrier will be required. Although travel will be allowed on this access route, travel will need to be kept to a minimum. Since this trail is currently closed, some light road maintenance may become necessary. This may be dependent on the amount of use the road receives and the types of equipment using it during implementation. A pre and post assessment of road conditions will be made by the COR, the vendor and the District Roads Engineer.

C4.5 Motorized travel beyond the intersection of 405.2B and 405.2C is currently prohibited. A physical barrier is currently in place blocking an old trail/road. The Forest Service will be responsible for removing and reinstalling this barrier both prior to and after completion of the project. Access will be granted for all vehicles to a meadow identified at the pre-bid meeting to facilitate fueling and servicing project equipment when completing the Mesa South and Mesa North Units. Access beyond this "meadow" will be limited to implementation equipment ONLY unless extraordinary circumstances arise. These situations must be coordinated with the COR/Inspector prior to use unless a situation is deemed as an emergency.

C.4.6 Periodic maintenance of the road/trail surface as well as maintenance, restoration of water bars or tank traps will be completed by the contractor prior to acceptance and payment of the project. This also includes access trails from the "meadow" to treatment units Mesa North and Mesa South. Post treatment seeding will be completed by the Forest Service.

C.5 GOVERNMENT FURNISHED PROPERTY:

The government will provide project maps and specifications to the Contractor(s) at the pre-bid meeting. There are no further Government furnished equipment or supply needs anticipated.

C.6 DELIVERY, STORAGE AND HANDLING:

The Contractor will provide transportation for equipment, operator, fuel and supplies to and from the site. The Contractor shall be responsible for providing the necessary material handling equipment to on-load and off-load materials as well as provide appropriate security for equipment left on site.

C.7 LUBRICATING, SERVICING AND REPAIRS OF EQUIPMENT:

The Contractor shall be responsible for the service and repair of all equipment including service and support vehicles. The use of welding equipment for equipment repair may be limited by fire restrictions. It is the Contractors responsibility to keep current on any current fire restrictions. The contractor shall keep the equipment in proper functioning condition to insure objectives for the project are met.

C.8 EVALUATION CRITERIA:

The following criteria will be used to evaluate and select a vendor for the implementation of this contract:

1) Past performance on similar projects involving mastication.

2) Due to the sensitivity of the soil profile and the need for minimal soil disturbance, a rubber tired or track system is required.
