REPAIR and/or REPLACEMENT OF ROOFING MATERIAL and ROOF
SHEATHING
Item No. 3A
REPAIR and/or REPLACEMENT OF ROOFING MATERIAL and ROOF SHEATHING
Contractor is responsible for troubleshooting the leak in the roof at the front entrance, assessing the extent of damage, furnishing new material, and installing the repair.
Preliminary assessment shows the leak is occurring where indicated on the drawings, is likely caused by breaks in the valley flashing and the EPDM membrane, and has resulted in the rotting of about 2 sq. ft of roof sheathing. It is thought that none of the structural members or any of the existing shingles need be replaced.
The contractor is responsible for doing their own assessment of the problem and coming up with a solution. Contractor is responsible to submit their plan for the repair and the material cut sheets to the CO for approval before work starts. Repair will also include trim and ceiling replacement on the porch and any required painting of the trim and ceiling replacement to match existing. All installation will be per manufacturer’s instructions, CO approvals, and sound construction techniques.
TROUBLESHOOTING OF THE EXISTING LULA ELEVATOR
Item No. 4A
TROUBLESHOOTING OF THE EXISTING LULA ELEVATOR
This contract item is for the troubleshooting of the existing elevator which is a Limited Use, Limited Access (LULA) Flexi-Lift model by Access Industries, Inc. with a GMV Hydro Lift
System T3010EN power unit.
The LULA hasn't been functioning for several years, and a preliminary assessment shows the cause to be an oil leak due to a faulty seal and not a malfunction of the mechanical and electrical systems.
It is the responsibility of the Contractor to verify and troubleshoot what the problem is. The Forest
Service has the original installation manual and plans. A Report of the Contractor's findings on the problems, suggested repair, and costs will be given to the CO for approval.
REPAIRING OF THE EXISTING ELEVATOR
Optional Item No. 4B
REPAIRING OF THE EXISTING ELEVATOR
At the convenience of the CO, the Contractor's troubleshooting report from Item No. 4A will be the basis of this optional Item No. 4B, Repair of the Existing Elevator. If the scope of the needed repairs is significantly different from the preliminary assessment by the Forest Service or if the original bid price on Item No. 4B is significantly different from the Report from Item No. 4A, this price may be renegotiated.
The Contractor will furnish and install all required repairs and required maintenance per the Item
No. 4A, so the elevator works.
All installation must be according to manufacturer’s instruction, these plans, sound construction techniques, and in a manner approved by the CO prior to construction. All design analysis, cut sheets, shop drawings, and product data will be submitted for approval by the CO. Contractor is responsible for scheduling work with the CO to minimize workplace disruption.

ANALYSIS OF THE EXISTING HVAC INSTALLATION AND
RECOMMENDATIONS FOR REPAIR
Item No. 5A
ANALYSIS OF THE EXISTING HVAC INSTALLATION AND RECOMMENDATIONS FOR
REPAIR
This contract item is for the redesign, analysis, and troubleshooting of the HVAC (heating, ventilation, and air conditioning) system. Preliminary assessment of the system shows the original plans were not followed; there are no as built plans; the system wasn't commissioned, there are no operation manuals; the lower level single zoned system is causing excessive temperature variations;
there are no balancing dampers in any ducts, the building's existing HVAC has never been balanced; the HRVs (hear recovery ventilation)have never worked; and outside air exchange only works when the main AHUs are running.
There appears to be sufficient capacity of the A/C and heating systems and the two zoned upper level give acceptable comfort; but the problem is the lack of zoning in the lower level, lack of ability of the entire system to be balanced, and lack of outside air exchange.
The Contractor is responsible for the full assessment of the system's condition and problems, and to work with the CO to assure all HVAC problems are fully addressed.
The analysis of the existing system will include an complete HVAC analysis according to all applicable ASHRAE and ICBO Standards and , including but not limited to total heating and cooling capacity, air flow requirements for each room, duct sizes, fire dampers, ability to balance the system, indoor air quality, and outdoor air rates. This analysis will include actual measurements of the existing system. This item will include recommendations for repair and modifications of the existing system. Such recommendations will include, but not be limited to recommendations of making two zones in the lower level with VAVs, balancing dampers, proper functioning of the HRV and IAQ systems, CO2 sensors, and energy conservation. A Report will be given to the CO for approval which includes the analysis, recommendations, sketches, proposed product data, and cut sheets.
REPAIR AND MODIFICATION OF THE EXISTING HVAC SYSTEM
Item No. 5B
REPAIR AND MODIFICATION OF THE EXISTING HVAC SYSTEM
At the convenience of the CO, the Contractor's Report from Item No. 5A will be the basis of Item
No. 5B. If the scope of the needed repairs is significantly different from the preliminary assessment by the Forest Service or if the original bid price on Item No. 5B is significantly different from the
Report from Item No. 5A, this price may be renegotiated.
Repair and modification of the existing system will be the furnishing and installation of all required material; balancing of the system, documentation by measurement of compliance with ASHRAE
62.1; providing of required documentation in ASHRAE 62.1, Sec. 7.2 including balancing, operating and maintenance manual, HVAC control sequences, diagrams, design criteria, and as built drawings of the system.
All installation must be according to manufacturer’s instruction, sound construction techniques, and in a manner approved by the CO prior to construction. All design analysis, cut sheets, shop drawings, and product data will be submitted for approval by the CO prior to construction.
Contractor is responsible for notification and approval by the CO of the proposed schedule and any limitation that repair of the HVAC will impose on the ability of the building to be used.
Optional Item No. 5C:
REPLACEMENT OF THE EXISTING FURNANCES AND AIR HANDLING UNITS.
The contractor will provide itemized prices for total replacement of the existing furnaces, IAQ and other HVAC systems. This option will include a cost benefit analysis of the proposed replacements.
Replacement furnaces will have a minimum AFUE of 90%, CO2 controls for IAQ, other HVAC, and other energy saving equipment.
All design analysis, cut sheets, sketches, and product data will be submitted for approval by the CO.
At the convenience of the CO, this Repair and Modification of the existing HVAC system may be negotiated as a substitute for Items No. 5B.

This contract item is for the furnishing and installation of insulation in the indicated walls.
The existing walls to have insulation installed are all 2x4 stud walls with finished 5/8" drywall.
Preliminary assessment by the Forest Service indicates that the best method to insulate these walls would be with blown or spray insulation through small holes drilled in the drywall.
The Contractor will submit, for approval, a work plan indicating the exact material and method used. Included will be recommendations, product data, shop drawings, cut sheets, and a plan and schedule. Contractor is responsible for working with the CO to schedule work and coordinating with the building occupants to minimize workplace disruption.
These submittals will be approved by the CO before construction.

Optional Item No. 8A
FURNISH, INSTALL, AND/OR REINSTALLATION OF ATTIC INSULATION
This optional contract item is for the furnishing, installation, and reinstallation of the ceiling insulation.
The building was designed for R40 ceiling insulation throughout. 12" of fiberglass batting was installed in most areas. Due to incomplete installation in the original construction contract in 2003, an estimated 1000 sq. ft. of this insulation was improperly installed or was not installed at all. Some of the improperly installed insulation may be reused and properly placed, and other insulation must be furnished. All installed insulation will be 12" of fiberglass batt unless insulation is proposed and approved by the CO. Installed insulation will envelop the entire area, and will be cut and fit tightly around obstructions, trusses, and ductwork throughout the ceiling area.
Access to the attic is through a 2'x3' hatch in Rm. 05.
