SCOPE OF WORK
DEMOLISH TWO RESIDENTIAL STRUCTURES
January 13, 2016
Contractor shall provide all materials, equipment, and labor to demolish two residential structures located at 12940 US Highway 441 North, Canal Point, FL 33438. All permits and required notifications shall be performed and paid for by the contractor. The house, constructed in 1934, is a 3,122 square foot 2-story neoclassical house with a wood balloon frame. The 2-story apartment structure, constructed in 1938, is 1,100 square feet with a wood siding exterior.
Details of the two structures are contained in Enclosure 1.
Contractor shall ensure compliance with Specification Section 01010 – General Requirements (Enclosure 2).
The two structures have asbestos containing materials. Contractor shall adhere to all requirements in Specification 02 08 10 – Engineering Control of Asbestos Containing Materials (Enclosure 3). The contractor shall use the attached hazardous materials assessment (Enclosure
4) to develop the method of disposing of and/or remediating any hazardous materials found at the site in accordance with all local, state, and federal regulations.
Due to the age of the buildings, lead based paint is also present. The contractor shall remediate and dispose of any lead based paint in accordance with all applicable laws and regulations and the hazardous assessment. The General Contractor shall make, in proper and timely fashion, any necessary notifications to relevant Federal, State, and local authorities and shall obtain and comply with the provisions of all permits or applications required by the work specified, as well as make all required submittals required under those auspices. The General Contractor shall indemnify the Government, their representatives and agents from, and pay for claims resulting from failure to adhere to these provisions. The costs for permits, applications, and the like, are to be assumed by the General Contractor.
Workers who will have the potential of lead exposure shall have proof of successfully completing a training course which covers the topics required by 29 CFR 1926.62. Contractors are also advised that training in other areas may be required by OSHA and are responsible to ensure that all training requirements for appropriate trades and procedures are met.
The General Contractor shall specify an on-site Supervisor or Competent Person who is fully qualified in all aspects of safe work practices and procedures with lead containing materials, and have (or will have) completed a training course within the previous year prior to the commencement of lead related work. The lead training course will cover all topics required by 29 CFR 1926.62 as well as training in relevant federal, state and local regulatory requirements, procedures and standards (including 454 CMR 22.00), supervisory techniques, and proper disposal procedures.
During demolition, provide safeguards, including warning signs, barricades, temporary fences, warning lights, and other similar items that are required for protection of all personnel during demolition and removal operations. Maintain fences, barricades, lights, and other similar items around exposed excavations until such excavations have been completely filled.
Completely demolish and remove buildings, structures and utilities, including all appurtenances related or connected thereto. Demolition shall include removing the buildings, foundations, existing septic tank, water line to the meter, and electrical service to the service pole.
Remove existing utilities and terminate in a manner conforming to the nationally recognized code covering the specific utility and approved by the Contracting Officer. If utility lines are encountered that have not been mentioned in this SOW, the Contracting Officer shall be notified prior to further work in that area.
Existing water line, approximately 50 feet, shall be removed to the meter located at the front of the property by the roadway. Water line between house and apartment, approximately 25 feet, shall also be removed. Cap water line at meter.
Removal of overhead electrical lines, approximately 80 feet and 90 feet, shall be coordinated jointly with the Government and local electrical utility, Florida Power and Light (FPL) (Account Rep, Kim Saunders, 941-637-5550), so that the utility can remove the wires.
The contractor may be required to disconnect the power at the meter can. The electric meters have been removed from the structures, however, the wires are still hot from the meter cans on both buildings to the pole located between this property and the next. The contractor shall ensure the wiring for the house next door remains intact.
Disturbed ground shall be leveled, seeded with Bahia, and mulched with an inch of straw to mitigate erosion. Excavated soil shall be used for backfill in the septic tank area(s). Any additional fill shall be acquired from the onsite stockpile. The contractor shall coordinate this with the site point of contact.
On completion of work, leave site in clean condition. Clean-up shall include off Government property disposal of all items and materials not required to remain property of the Government as well as all debris and rubbish resulting from demolition operations.
