Project Title: Middle Kyle Administrative Site Water System

Purpose of Amendment: To replace Project Specifications, answer questions posed by vendors, and provide 09-25-12 Site Visit Notes and Attendees list.

Receipt of Offers: The date, time, and receipt of proposals remains unchanged. The place for receipt of offers remains the same.

Acknowledgment: See Block 11 above regarding how to acknowledge this amendment. The acknowledgement must be received at the place designated for receipt of offers (see Block 19 of the Solicitation, Offer, and Award, Standard Form 1442).

Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

3. Other Question and Answers:

Q1 - We are considering bidding the Middle Kyle Admin. Site Water Improvement project. Our company is based out of another state, we were wondering if we need to file for a business license in the state of Nevada in order to do this project or if since it is a federal project we can do work in any state.

ANSWER: You are not required to have a Nevada license, but should be licensed in your home state.

Q2 -. On the Schedule of items 017320-6 Horse Structure Demolition. I don’t recall seeing horse structures on the site visit. Have these already been demolished? It also lists these structures to be demolished on sheet C6 of the drawings.

ANSWER: The horse structures have been demolished. The work will consist of clearing the manure piles off site and site preparation as discussed to the show me trip. Offerors will need to bid for only the manure disposal and site grading.

Q-3 – Are there any soils surveys or reports for this project?

ANSWER: No.

Middle Kyle Water System Pre-Bid Conference

Date: Tuesday, September 25th, 2012

Place: Meeting Place Well #1 near Mile Post 5 Hwy 157

Time: 10:00 AM

A. Objective The purpose of this meeting was to familiarize potential contractors to the site, walk the terrain, identify key components of the Middle Kyle Water System construction project and respond to questions about the solicitation.

B. Agenda

1. Introductions – Lines of Authority on this Contract

a. Marge Schafer is the Contracting Officer on the project. She is the point of contacts for questions on the solicitation or PS&E.

b. Steven O’Neil is the designer of the project as well as the Contracting Officer’s Representative.

2. Contract Dates

a. Contract time – 120 days

b. Questions for Solicitation close – October 11th

c. Project Proposals due – Oct. 18th

d. Amendment to sheets 28 and 29 of plans will be posted on the solicitation

e. Contractor is responsible for all permits

f. USFS has State Water Permits and will provide to awarded contractor.

3. Key Components and Questions

a. Well #1

• Scarify location of old corral and components around well #1 up to 4” in depth and haul manure off site and dispose of in a legal manner as identified by Steven O’ Neil. Reshape the natural ground and berm to slope to grade.

• Pump house will be furnished by the government, but the pad and stub outs for the pump house will be prepared by the contractor as a part of this contract.

• Contractor will coordinate electricity disconnect and connections.

• Does the contractor contact NV Energy?

o Yes. That will be the responsibility of the contractor.

• Do we leave the slope of the floor flat?

o CXT will slope the floor.

o The contractor will need to coordinate with the COR for delivery date and installation from CXT.

• Removal and replacement of well pump, discharge line and electrical at well #1

• Demo old well house and remove from site.

• Other contracts in the area will require use of the water supplied by Well #1; the contractor will provide a connection point at the well site or another approved location for construction water.

b. Existing sleeve under Highway 157

• The existing sleeve under highway shall extend past edge of pavement per drawings.

• The existing sleeve is 10” diameter material.

• Is the widening of Highway 157 a part of this contract?

o No. That will be done under a separate contract.

• Do you know exactly where the sleeve is?

o No, but the USFS will look into existing as-builts for staking.

o Contractor will locate the sleeve and cut to appropriate length.

c. Archeological sites and monitor

• An archeological monitor will be present to observe excavations and trenching from well #1 to the sleeve tie-in at Highway 157. The main focus area will be from the main line to the visitor center site. See Sheet 24 (W12) for locations.

• Clearing and Grubbing in this area:

o The clearing and grubbing in the area identified on Sheet 24

(W12) will strictly adhere to the existing disturbed road way.

o The contractor shall use a bucket no wider than 24” to perform the trenching from the tee on the main to the visitor center site;

STA 0+00 – 4+64.

o Utilize narrow trenching techniques and if walls of trench have to be sloped back, contractor must first consult with COR.

o The contractor must make every effort to minimize the disturbed area.

o In the event that archeological findings are made, the contractor will stop work immediately in the area and notify the

COR.

• The monitor will be coordinated by the COR.

• Is the alignment of the road going to remain unvegitated?

o Yes, for the most part it will be the way you see it now. If there is a call out for wood chips (Well #1 to Sleeve), the USFS will provide wood chips and contractor will need to follow the specs for placement.

d. Private access road to ponds

• The access road is a private road located behind the church across the highway from the Mount Charleston Resort on the northern side of Highway 157.

• If road must be widened, cut into the hill and away from the church property, making efforts to protect the church’s outside area.

• Contractor to coordinate with the Church’s representatives.

e. Waterline trenching to future administrative site

• 6” line to future admin site with a daylight drain

• Some portions of trenching will share 2” and 6” line – see plans.

• 4’ minimum burial depth for entire length of project (i.e. waterline, valves, etc.)

• Daylight drain at the end of the line. East-most part of the waterline.

• Will run in front of the NDOT and Metro Police Station

• Coordinate patch and trench to minimize trench disturbance in front of

NDOT station

• Does that apply to transfer stations too?

o Yes.

• Are the asphalt trenches in the NDOT ROW?

o Yes, and the ROW asphalt specs are in the plans.

• What is the timing on the asphalt patch?

o Depending on temperatures and the weather, the patch may have to go in next Spring. Check the specs for hot or cold mix asphalt.

• Is there an air release valve at the end of the waterline?

o No, just a daylight drain to prevent holding pressure while the line is not yet in use.

f. Demolition and abandonment of Well #2

• In the valley of the old golf course behind the Mount Charleston Resort.

• Per NAC requirements and other legal regulations

• The USFS will provide access to the well.

g. Demolition of ponds and tanks

• Northeast pond and tanks to be completely removed from site and replace with new tanks at these locations

• Southwest and middle pond, concrete can be crushed but must be permeable

• Lower existing Road by 6’ to generate fill material for ponds

• Can we use same dirt/material for backfill of tanks?

o If it meets the specs for the new tank’s manufacturer spec for fill material gradation, then yes.

• Is there a max width with digging the trench?

o The clearing and grubbing limits shall not be more than 12’ wide, with as minimal disturbance as possible to provide access to the trench with equipment.

• When they cut down this area and start filling in the pond, will it be a compacted fill that has to be tested?

o Yes, it will be compacted and there will be voids because of the crushed concrete. Percent compaction is called out in the specs.

• Do we need to do 3 tests per project for the asphalt?

o Bid how it is specified in the plans, per measurement of asphalt.

Section 312225 3.11-C specifies the frequency of the testing requirements.
