B. General Instructions/Information.
1. The selection of Offeror(s) for award purposes will be conducted utilizing competitive source selection procedures as delineated in FAR Part 15.3. Noncompliance with the solicitation requirements may hamper the Government's ability to properly evaluate the proposal and may result in elimination of the proposal from further consideration for contract award. This is a competitive best value source selection in which competing Offerors' past performance history will be evaluated on a basis significantly more important than cost or price considerations. It is the Government’s intent to award a contract to the Offeror who will provide the best overall value to the Air Force as determined by an integrated assessment of the evaluation criteria in the addendum to provision FAR 52.212-2 – Evaluation of Commercial Items, Attachment 2, of this solicitation. Technical capability will be evaluated and given a rating of acceptable or unacceptable; unacceptable offers are not eligible for award.
2. The Government intends to award up to three (3) Indefinite Delivery Indefinite Quantity (IDIQ) contracts and an initial Task Order (TO) for the West Region as a result of this solicitation. There is a potential there will not be any additional TOs awarded against these IDIQ contracts, following award of the three Regional (West, South, and North) TOs.
3. Award of the basic IDIQ contracts will be based on the proposals submitted for the task order for the West Region. Regions are listed in Exhibit 1 - Military Treatment Facilities (MTFs) by Region, in Attachment 2, of this solicitation. Following the post award conference, a TO will be issued for the West Region amongst the IDIQ awardees. The Government will then make a separate request for proposal for the North and South Regions. Award of all TOs will be made utilizing the past performance trade-off methodology. Offerors shall not submit pricing for the North and South Regions as part of this solicitation.
http://www.fbo.gov/ mailto:anthony.hill.26@us.af.mil
4. The Contracting Officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the Contracting Officer will review this determination and if, in the Contracting Officer's opinion, adequate price competition exists, no additional cost information will be requested, and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the Contracting Officer determines that adequate price competition no longer exists; Offerors may be required to submit cost data to the extent necessary for the Contracting Officer to determine the reasonableness of the price.
5. To assure timely and equitable evaluation of the proposal, the Offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The proposal should not simply rephrase or restate the Government's requirements but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to the identified evaluation factors and sub-factors in the addendum to provision FAR 52.212-2 – Evaluation of Commercial Items, Attachment 2, of this solicitation.
Failure to meet a requirement may result in an offer being ineligible for award. The proposal (offer) shall consist of four (4) separate Volumes; Volume I – General, Volume II – Technical Capability, Volume III – Past Performance Information, and Volume IV – Price.
6. The submission of the documentation specified below will constitute the Offeror's acceptance of the terms and conditions of the solicitation, concurrence with the Performance Work Statement (PWS) - Attachment 4 of this solicitation, and contract type. A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful Offeror within the time for acceptance specified in the offer, shall result in a binding agreement without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer, whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received from the Offeror before award.
7. It is the Government’s intention to award without discussions. Offerors are encouraged to present their best technical proposal and prices in their initial proposal submission. However, in accordance with (IAW) FAR Part 15.306, should discussions become necessary, the Government reserves the right to conduct them. If this occurs, a competitive range will be determined and Offerors will be notified. The competitive range may be limited for purposes of efficiency IAW FAR Part 15.306 (c)(2). If the Government awards without discussions, the Offeror’s may be given the opportunity to clarify certain aspects of proposals or to resolve minor clerical errors.
The Government may reject any or all offers if such action is in the government’s interest.
8. Instructions outlined in section C. below, prescribe the format for the proposal and describe the approach for the development and presentation of proposal data. These instructions are designed to ensure the submission of necessary information to provide for the understanding and comprehensive evaluation of proposals.
9. If an Offeror believes that the requirements in these instructions contain an error, omission, or are otherwise unsound, the Offeror shall immediately, but no later than the set time for the initial submission of proposals, notify the Contracting Officer in writing with supporting rationale as well as the remedies the Offeror is asking the Contracting Officer to consider as related to the omission or error. The Offeror is reminded that the Government reserves the right to award this effort based on the initial proposal, as received, without discussion.
10. In accordance with FAR Subpart 4.8 (Government Contract Files), the Government will retain one copy of all unsuccessful proposals. Unless the Offeror requests otherwise, the Government will destroy extra copies of such unsuccessful proposals.
11. All referenced documents for this solicitation are available on the Federal Business Opportunities (FedBizOpps) website at www.fbo.gov.
12. The Contracting Officer will promptly notify Offerors of any decision to exclude them from the competitive range, whereupon, they may request and receive a debriefing in accordance with FAR 15.505. The Contracting Officer will notify Offerors who were in the competitive range, but were not selected for award in accordance with FAR 15.503(b). Upon such notification, unsuccessful Offerors may request and receive a debriefing in accordance with the requirements of FAR 15.506.
13. The Government reserves the right to cancel this solicitation, either before or after the closing date for receipt of proposals. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an Offeror for any costs.
C. Proposal Preparation Instructions.
1. The proposal instructions in this section are designed to assist Offerors in preparing a complete response that reflects a full understanding of the approach proposed to accomplish all the requirements as stated in the PWS and the evaluation criteria in the addendum to provision FAR 52.212-2 – Evaluation of Commercial Items, Attachment 2, of this solicitation. 4. Proposal Content
(a) Volume I – General. Offerors are required to submit a completed SF 1449 (Solicitation, Offer and Award) (including acknowledgment of Amendments) and complete FAR provision 52.212-3, Offeror Representations and Certifications – Commercial Items (Nov 2014). Failure to follow the below proposal preparation instructions may cause your proposal to be deemed unacceptable by the Government.
The General volume shall be organized as follows and contain the identified information.
TAB A, Solicitation, Offer and Award – Complete blocks 12, 17a (including CAGE code), and 30a, b, and c of the Standard Form (SF) 1449. In doing so, the Offeror accedes to the contract terms and conditions as written in the solicitation/Request for Proposal (RFP), with attachments. The solicitation constitutes the model contract. Provide a signed SF 1449 and signed SF 30/amendments, if applicable. All solicitation amendments must be acknowledged in accordance with FAR 52.212-1, Instruction to Offerors- Commercial Items and submitted with the proposal. The Offeror shall provide the name, title and telephone number of the company/division point of contact regarding decisions made with respect to your proposal and who can obligate your company contractually. Also, identify those individuals authorized to negotiate with the Government.

Note: The Offeror shall complete the Pricing Matrix, Attachment 3, of this solicitation in lieu of completing Section B (Supplies or Services and Prices/Costs) of the SF 1449.
(b) Volume II – Technical Capability. The technical approach will be evaluated for evidence of the degree to which the Offeror demonstrates a clear understanding of the requirements for Referral Management Services as defined in the PWS. Responses should display a reasonable, well-thought-out approach that demonstrates the Offeror's understanding of tasks to be performed, as well as the technical approach and methodology for accomplishing tasks under any resultant award to meet the requirements of the PWS.
a. The Technical Volume shall be clear, concise, and include sufficient detail for effective evaluation and for substantiating the validity of stated claims in the Offeror’s proposal. Legibility, clarity and coherence are very important.
Responses will be evaluated against the technical factors defined in the addendum to provision FAR 52.212-2 Evaluation – Commercial Items, Attachment 2, of this solicitation. The proposal should not simply rephrase or restate the Government's requirements, but rather shall provide convincing rationale to address how the Offeror intends to meet these requirements. Statements that the Offeror understands, can, or will comply with the PWS (including referenced publications, technical data, etc.); statements paraphrasing the PWS or parts thereof (including applicable publications, technical data, etc.); and phrases such as “standard procedures will be employed” or “well known techniques will be used,” etc., will be considered unacceptable. Offerors shall assume that the Government has no prior knowledge of their facilities and experience, and will base its evaluation on the information presented in the Offeror's proposal.
Elaborate brochures or documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired.
b. The Technical Volume shall, at a minimum, be prepared in a form consistent with the PWS and the evaluation criteria for award set forth in the addendum to provision FAR 52.212-2 Evaluation – Commercial Items, Attachment 2, of this solicitation. The section shall be prepared in an orderly format and in sufficient detail to enable the Government to make a thorough evaluation of the Offeror’s technical competence and ability to comply with the contract task requirements specified in the PWS. The Offeror shall address as specifically as possible the actual methodology to be used in accomplishing the PWS tasks. The volume shall be organized according to the following general outline:
• Table of Contents
• List of Table and Drawings
• Evaluation Criteria
• Factor 1 – Technical Capability o Sub-Factor 1 – Staffing Plan o Sub-Factor 2 – Training Plan o Sub-Factor 3 – Operations Plan o Sub-Factor 4 – Contingency Plan o Sub-Factor 5 – Transition Plan
(c) Volume III - Past Performance Information. The Past Performance evaluation will be accomplished by reviewing aspects of an Offeror's recent and relevant past performance, focusing on and targeting performance which is relevant to the effort, as it directly relates to the work being procured under this solicitation and as defined in the addendum to provision FAR 52.212-2 Evaluation – Commercial Items, Attachment 2, of this solicitation. The evaluation of past performance information will take into account past performance information regarding subcontractors, teaming partners, and/or joint venture partners that will perform critical aspects of the requirement when such information is relevant to the work being procured under this solicitation. Only references for recent, relevant contracts are requested.
a. The Government seeks relevant past performance information from the Offeror. The Offeror must provide no more than three (3) of the most relevant contracts performed for Federal agencies and/or commercial customers within the last three (3) years from the date of issuance of this solicitation.
To the extent the Offeror and its subcontractors (to include, teaming partners and/or joint venture partners), possess relevant past performance, Offerors are required to submit no more than three (3) relevant past performance efforts. If the Offeror intends to utilize a subcontractor, teaming partner, and/or joint venture partner in the performance of this effort, one (1) of the relevant efforts can be from a subcontractor, teaming partner, and/or joint venture partner. If the Offeror has only (1) relevant past performance effort, then two (2) of the relevant efforts can be from a subcontractor, teaming partner, and/or joint venture partner, the Offeror will use on this contract. If the Offeror has no relevant past performance efforts, then all three (3) of the relevant efforts can be from a subcontractor, teaming partner, and/or joint venture partner, the Offeror will use on this contract.
In all cases, the Offeror shall include documentation regarding their relevant past performance as it directly relates to the work being procured under this solicitation. The Offeror shall also submit the role of the prime, the identity of team members and the percentage of the proposed effort that is to be performed.
Past performance of potential subcontractors, teaming partners, and/or joint venture partners, if applicable, shall be submitted as part of the Offeror’s proposal.
b. In the event an Offeror is not able to provide any documentation regarding relevant past performance within the last three (3) years, then the Offeror shall provide a statement to that effect. If an Offeror provides the aforementioned statement, and it is later discovered by the Contracting Officer that an Offeror failed to disclose a relevant contract, the Contracting Officer may find the Offeror noncompliant.
c. For each identified effort for a commercial customer, the Offeror shall also submit a client authorization letter, authorizing release to the Government of requested information on the Offeror's performance.
d. If the Offeror intends to utilize a subcontractor, teaming partner, and/or joint venture partner in the performance of this effort, then the Offeror shall submit, a consent letter, executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing release of adverse past performance information to the Offeror so the Offeror can respond to such information.
In your selection of recent and relevant projects/programs, consider the following:
Recency: Is a measure of the time that has elapsed since the past performance reference occurred. Recency is generally expressed as a time period during which past performance referenced are considered relevant.
For the purposes of evaluation under this solicitation, the relevant effort must be ongoing or must have been performed within the past three (3) years from the date of issuance of this solicitation. The ongoing and/or performed effort must be at least 12 months in duration.
Relevancy: To be relevant, past performance must include efforts involving Referral Management Services that are similar in scope, magnitude, and complexity to efforts this solicitation requires. Table 2 – Relevancy Determination, of addendum to provision FAR 52.212-2 Evaluation – Commercial Items, Attachment 2, of this solicitation illustrates how the Government will assess past performance with regard to scope, magnitude and complexity.
e. In conducting the past performance evaluation, the Government reserves the right to use both the information provided in the Offeror’s Past Performance Volume and information obtained from other sources, such as the Past Performance Information Retrieval System (PPIRS), the Contract Performance Assessment Reporting System (CPARS), Defense Contract Management Agency (DCMA) or similar systems, and commercial sources. Offerors are reminded that both independent data and data provided by Offerors in their proposals may be used by the Government to evaluate Offeror past performance. However, the burden of providing thorough, complete, and current past performance information as requested in this paragraph remains with the Offerors. Proposals that do not contain the information requested, risk being rejected or receipt of an unacceptable performance rating.
f. Submission Requirements. The offeror shall submit a Past Performance
Volume containing the following:
• Sample Client Authorization Letter – see Exhibit 1. For each identified effort for a commercial customer, the Offeror shall submit a Client Authorization Letter, authorizing release to the Government of requested information on the Offeror's performance.
• Sample Subcontractor/Teaming Partner Consent Letter - see Exhibit 2.
The Offeror shall provide a consent letter executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing the release of past performance information so the Offeror can respond to such information.
• Summary Page. The Offeror shall describe their role (as prime, subcontractor, teaming partner, and /or joint venture partner) for each effort identified in Exhibit 3, Relevant Contracts Reference Sheet. The summary page for each relevant contract referenced shall not exceed one (1) page.
• Relevant Contracts Reference Sheet – see Exhibit 3. The Offeror shall provide documentation outlining the Offeror’s past performance with contracts, as prime, subcontractor, teaming partner, and /or joint venture partner, which is the same or similar in nature, size, and complexity to the services being procured under this solicitation. The submittal shall include rationale supporting the Offeror’s assertion of relevance and how it was determined that the work performed previously was the same or similar in nature, size, and complexity to the work specified by this solicitation.
Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort, and to what aspects of the proposed effort they relate. Offerors are limited to a maximum of three (3) pages per reference for each relevant contract.
• Past Performance Questionnaire – see Exhibit 4. For the contracts identified on each Relevant Contracts Reference Sheet, the Offeror shall forward a copy of the past performance questionnaire to the points of contact responsible for monitoring performance under such contracts. The points of contact shall return the questionnaires via email directly to the Contract Specialist (Mr. Anthony Hill) at anthony.hill.26@us.af.mil, not later than the solicitation due date, noted in paragraph A.1. Any questionnaires not returned directly to the Government point of contact (Mr. Hill) will not be reviewed and evaluated. The information contained in the questionnaires will be used to evaluate the Offeror’s past performance. New entities that have no past customers shall annotate on the Summary Page that they have had no previous clients and that the minimum number of questionnaires cannot be provided. A sample Past Performance Questionnaire cover letter is included as Exhibit 5.
• Organization Structure Change History - many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant present or past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" (limited to two (2) pages) describing all such changes in the organization of your company. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation assessment. Since the Government intends to consider present and past performance information provided by other sources as well as that provided by the Offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.
(d) Volume IV- Price – The Offeror shall complete the Referral Management Pricing Matrix, Attachment 3, of this solicitation in lieu of completing Section B of the SF 1449.
The Government will use this information (Attachment 3) as part of the Offeror’s proposal to evaluate price reasonableness, realism, balance and completeness.
a. The price under this solicitation shall be expressed as the extended amount, which is equal to the quantity of units multiplied by the unit price.
b. Offerors shall follow the instructions within the Referral Management Pricing Matrix, Attachment 3, of this solicitation.
c. The Government reserves the right to require the submission of Other Than Certified Cost or Pricing Data as is deemed necessary to arrive at a fair and reasonable price.
(End of Provision)
EXHIBIT 1
SAMPLE CLIENT AUTHORIZATION LETTER
Past performance information concerning private sector contractors, subcontractors and joint venture partners cannot be disclosed to the Government without their consent. Client authorization letters are required for each identified effort for a commercial customer. This letter will authorize release to the Government of requested information on an Offeror's performance.
The Government will need that consent before contacting commercial customers to assess an Offeror’s past performance. In an effort to assist the Government's Past Performance evaluation team in assessing your past performance relevancy, we request that the following client authorization letter be completed by any commercial customers identified in your proposal. The completed client authorization letters should be submitted as part of your Past Performance Volume III Proposal.
Offerors should send with their list of references a letter similar to the following authorizing the reference to provide past performance information to the Government.

We have identified Mr./Ms.____________________________ of your organization as the point of contact based on his/her knowledge of our work. Your cooperation is appreciated. Any questions may be directed to .
Sincerely, EXHIBIT 2
SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE
OF PAST PERFORMANCE INFORMATION TO THE PRIME CONTRACTOR
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government's Past Performance Evaluators in assessing your past performance relevancy, we request that the following consent form be completed by the subcontractors, teaming partners, and joint venture partners identified in your proposal. The completed consent forms should be submitted as part of your Past Performance Volume III Proposal.

4. Relevancy Description. Identify in specific detail for each contract listed, why or how you consider that effort relevant or similar to the effort required by this solicitation. In determining relevancy, consideration should be given, but not limited to, such things as product/service similarity, product/service, size and complexity, contract type, contract environment, division of company proposing, and subcontractor interaction.
5. Narrative Explanation. On each contract listed describe the objectives achieved and the following data: any cost growth or schedule delays encountered. For any Government contracts which did not/do not meet original requirements with regard to cost, schedule, or technical performance, a brief explanation of the reason(s) for such shortcomings and any demonstrated corrective actions taken to avoid recurrence. Offerors may include a discussion of efforts accomplished by the Offeror to resolve problems encountered on prior contracts as well as past efforts to identify and manage program risk. Merely having problems does not automatically equate to a “Less than Satisfactory” rating, since the problems encountered may have been on a more complex program, or an Offeror may have subsequently demonstrated the ability to overcome the problems encountered. The Offeror is required to clearly demonstrate management actions employed in overcoming problems and the effects of those actions, in terms of improvements achieved or problems rectified.
6. Negative Performance Documentation. A copy of any cure notices, show cause letters or contract discrepancy reports received on each contract listed and a description of any corrective action by the Offeror.

FOR OFFICIAL USE ONLY
The 773d Enterprise Sourcing Squadron is in the process of selecting a contractor for the Referral Management Services program. Referral Management (RM) refers to the process by which primary care providers refer patients to specialty care. When specialty care is referred, the Referral Management Center (RMC) is responsible for advising the patient of their benefits, working with the referring provider and/or the Managed Care Support Contractor (MCSC) to ensure that the referral is accurate, coordinating the referral with the specialty office, and returning all results of treatment to the patient’s medical record.
One of the considerations in proposal evaluation is the verification of the Offerors' past and present performance on contracts, which reflect the Offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an Offeror, for the evaluation of the Offeror's performance on those contracts.
Our areas of interest are summarized in the enclosed questionnaire. Given that our schedule is extremely tight, we respectfully request your response no later than the due date of the solicitation, (to be inserted by the Contractor).
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out “by hand” and emailed to the Contract Specialist (Mr. Anthony Hill). You may confirm receipt of your questionnaire by requesting a return email. A copy of the returned email, along with associated metadata, shall be proof of receipt of the question. Your completed questionnaire will become a part of the official Source Selection records.
Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
