About this file

The document is an Affirmation and Disclosure Form related to a state contract opportunity, designed to ensure compliance with executive orders regarding offshore services and data location. The form requires contractors to disclose the principal location of their business, the locations where services will be performed, and where state data will be stored, both for the primary contractor and any potential subcontractors. Contractors must affirm that they have read and understand prohibitions on performing offshore services, locating state data offshore, or purchasing from Russian institutions or companies.

The form stipulates that contractors have a continuous duty to disclose any changes in service locations before, during, and after contract execution, with the state retaining the right to immediately terminate the contract unless a signed waiver is obtained to perform services outside the United States. Failure to provide complete location information may result in no contract award. The document includes signature lines for the contractor to acknowledge understanding of these requirements, signifying that this form is an integral part of any potential state contract and will be incorporated into the final agreement.
