RE: Final Report‐ Pre‐Demolition Services LEGEND No. 23WI059
1.0 INTRODUCTION
This report presents the results of the pre‐demolition inspection of the Marshfield Medical Center located at 900 College Avenue W in Ladysmith, WI conducted by LEGEND Technical Services, Inc.
(LEGEND) representative Mr. Lucas Sokol, WI Asbestos Inspector #AII‐267101 and Katia Castro Lopez, WI Asbestos Inspector #AII‐290508 on May 2, 3, 13 and 24; June 7, 29, 2023. LEGEND’s services were requested and authorized by Gary Shilts with Rusk County.
The structure consist of three vintages of construction. This includes four‐story building built in 1947 (5 levels), a 1962 addition (3 levels), and a 1974 addition (3 levels). The exterior of all vintages was brick with flat membrane roofs throughout. Interior floor finishes included floor tile, ceramic tile, wood, carpet, and vinyl/ linoleum. Interior wall finishes included plaster, drywall, and concrete. Interior ceiling finishes included drywall, plaster, and ceiling tile.
At the time of inspection, the building was vacant and the electrical systems were energized.
2.0 SIGNIFICANT LIMITATIONS AND ASSUMPTIONS
LEGEND’s scope of work for this pre‐demolition inspection was limited to an asbestos containing materials (ACM) inspection which included the following significant limitations and assumptions:
Limitation Discussion
Foundation Coatings Reasonable attempts were made to identify and sample exterior foundation coatings. However, it remains possible that such coatings are present well below ground level and will be discovered only once heavy equipment has conducted significant excavation. ACM foundation coatings were identified in one area on the interior and are assumed to be present throughout.
Electrical System Until the electrical systems are de‐energized, safe inspection is not possible. Most electrical components contain other hazardous materials which require removal prior to demolition. It remains possible that suspect asbestos insulators are present in electrical system components.
Exhibit B

3.0 ACM INSPECTION
The US EPA, US OSHA, WI DHS, and WI DNR definitions of ACM state that any building material containing greater than 1% asbestos by volume is classified as an ACM. The US EPA, WI DHS, and WI DNR definitions also require that any material determined to contain asbestos less than or equal to 1% by volume be considered an ACM unless the result is confirmed by point counting methods. For materials in which multiple samples have been collected, only one sample needs to result as an ACM to consider all the material an ACM. LEGEND is licensed by the State of Wisconsin as a Certified Asbestos Company (#CAP‐15140).
A total of 453 bulk samples were collected from suspect ACMs. Laboratory analysis indicates 99 of these samples resulted >1% for asbestos; the results are summarized in the following tables:

4.0 SAMPLING STRATEGIES/METHODOLOGIES
An ACM inspection within the scope of work was conducted; suspect asbestos containing materials were sampled and shipped to LEGEND’s laboratory located at 88 Empire Drive in Saint Paul, MN 55103. Our laboratory is accredited by the National Institute for Standards and Technology (NIST), an agency of the US Department of Commerce. Our National Voluntary Laboratory Accreditation Program (NVLAP) number is 102081. The laboratory proficiency records, certifications, and quality assurance manual are available upon request.
Analysis for the presence of asbestos fibers in bulk samples was performed using polarized light microscopy (PLM) and dispersion staining techniques with an Olympus BHSP microscope at 40‐ 200X magnification in with current U.S. Environmental Protection Agency (USEPA) protocols, 40 CFR Appendix E to Subpart E of Part 763, “Interim Method of the Determination of Asbestos in Bulk Insulation Samples” and “Method for the Determination of Asbestos in Bulk Building Materials,” 1993 (EPA 600/R‐93‐116).
5.0 REMARKS
Reasonable attempts were made to inspect concealed areas, including mechanical equipment interiors, electrical equipment interiors (including wire insulation), paneling/fixture adhesives, CMU cavities, fire door interiors, ceramic materials, chimney interiors, and interstitial cavities, etc.
If any suspect ACMs not identified in the attached tables are discovered, work should be halted, and a State of Wisconsin Certified Asbestos Inspector should be contacted to determine the asbestos content of the materials.
Until removal, all maintenance persons and contractors entering the site should be made aware of the ACMs in the structure and provided a complete copy of this report.
WI DNR publishes WA‐651, “Planning Your Demolition or Renovation Project: A Guide to Hazard Evaluation, Recycling, and Waste Disposal”. This document provides guidance for the proper identification of the common harmful materials found on project sites; including asbestos, chlorofluorocarbons and halons, lead, mercury, polychlorinated biphenyls, and others. This document is accessible at https://dnr.wi.gov/files/pdf/pubs/wa/wa651.pdf.
WI DNR provides guidance for the proper handling of building debris from renovation or demolition activities at https://dnr.wi.gov/topic/demo/structures.html. This guidance includes the statements: “you could face fines and penalties if you burn a structure” and “in general, it is illegal to bury waste on your property”.
Prior to the project, notification to WI DNR and/or WI DHS may be made through the Asbestos Renovation/Demolition Notification System, accessible at https://dnr.wi.gov/topic/Switchboard.
6.0 RECOMMENDATIONS
Any disturbance to known or assumed ACMs in the structures should be performed in accordance with the various asbestos regulations including, but not limited to: WI DHS 159; WI DNR 447; US OSHA 29 CFR 1926.1101; US EPA 40 CFR 61 Subpart M; et al.
For demolition activities, the ACMs should be removed by a Wisconsin State Certified Asbestos Contractor employing trained and Wisconsin State Certified Asbestos Workers and Supervisors, prior to any disturbance.
CAT I ACMs in good condition may remain in place during normal mechanical demolition;
however, the demolition debris must not be crushed, ground, recycled, or otherwise rendered friable and must be properly disposed of in an approved landfill.
National Fire Protection Association (NFPA) regulation 1403 2‐2.10(j) states, “All forms of asbestos deemed hazardous to personnel shall be removed by an approved asbestos removal contractor” prior to a fire training burn.
7.0 STANDARD OF CARE
The recommendations in this report represent our professional opinions. These opinions were arrived at in accordance with currently accepted industrial hygiene consulting practices at this time and location. Other than this, no warranty is implied or intended. Any unconsumed portion of the analyzed samples will be retained in our laboratory for a period of seven days from the date of this report, unless prior instructions are received from the client.
