About this file

This document is an ADSD Work Plan Template Form issued by the Nevada Department of Human Services, Aging and Disability Services Division (ADSD) as part of a competitive Notice of Funding Opportunity for Supportive Services for Older Adults and Caregivers. The template provides the structured framework that applicants must complete when submitting applications for one of five service categories: Transportation ($2.1 million), In-Home Services ($4.2 million), Caregiver Support Services ($2.7 million), Food Security ($300,000), and Health Promotion Services ($230,000), representing a combined funding opportunity of approximately $9.5 million. The Work Plan requires applicants to detail three goals (Outreach, Service Delivery, and Other) with corresponding objectives, activities/strategies, timelines, and evaluation tools for each. The application deadline is March 20, 2026, at 11:59 p.m. (PT), with funding decisions announced in June 2026. An optional virtual Applicant Informational Meeting will be held on February 20, 2026, at 1:00 p.m. via Microsoft Teams. The project period extends from July 1, 2026, through June 30, 2028, with the budget period covering July 1, 2026, through June 30, 2027.

The Work Plan template requires applicants to establish measurable projections and expected outcomes, including the number of events, people reached, unduplicated clients served, units provided, and percentage of underserved populations served. Funding sources include federal Older Americans Act Titles III-B, III-D, and III-E; Social Services Block Grant (Title XX); Fund for a Healthy Nevada Independent Living Grant funding; and State General Funds. A 15 percent matching funds requirement applies, which may consist of non-federal cash or in-kind contributions. Applications are evaluated on a 50-point scoring matrix assessing Project Relevance and Priority Populations (10 points), Capacity and Approach (15 points), Cost Effectiveness and Sustainability (10 points), Project Impact (10 points), and Adherence to Application Instructions (5 points). Priority populations include individuals age 60 and older who are frail, homebound, or isolated; those at risk for institutional placement; individuals with Alzheimer's disease or dementia; those with greatest economic need; those with greatest social need; and those residing in rural, frontier, and tribal areas. Funding will be dispersed on a reimbursement basis, and ADSD reserves the right to reallocate funds if subaward terms are not met, though organizations will be provided 60 days to develop and implement a corrective action plan before potential fund reduction or termination.
