About this file
This document is a set of mandatory forms for a City of Newark, New Jersey contract opportunity involving Youth and Adults Development and Violence Prevention Programs. The forms include a Certification of Compliance with the Charitable Registration and Investigation Act, a Certificate of Compliance with Contractor Pay-to-Play Reform Ordinance, a Hold Harmless Agreement, and a Disclosure Statement for Nonprofits. These documents are standard compliance and authorization forms that bidding organizations must complete and submit as part of their proposal package for the contract opportunity.
The forms require bidding organizations to certify their compliance with various local regulations, including charitable organization registration requirements, campaign contribution restrictions, and organizational disclosure details. The Hold Harmless Agreement specifically requires the contractor to defend and indemnify the City of Newark against any claims or actions arising from the contractor's work. The Disclosure Statement requests information about the nonprofit's incorporation, principal place of business, and officers/directors. These forms are designed to ensure transparency, legal compliance, and risk mitigation for the City of Newark when entering into a contract for youth and violence prevention programs.
