About this file
This Additional Information document specifies printing requirements for the Wisconsin Home Owner Certificate for Manufactured Homes (form SBD-10680) for the Department of Safety and Professional Services. The contract involves printing 10,000 two-sided certificates at 8.5" x 11" size using specialized security features and materials. The initial contract term runs for one year from May 10, 2017 through May 9, 2018, with two automatic one-year renewal options, with a maximum potential expiration date of May 9, 2020. The contract will automatically renew unless either party provides written notice 30 calendar days prior to expiration. Email bid responses are preferred.
The stock must be 24-pound recycled, dull white bond paper with no optical brighteners, fully sensitized on both front and back to resist chemical and solvent eradication including acids, bases, bleaches, and petroleum products. Printing requires heat-resistant and erasable inks in specified PMS colors: black and PMS 185 red with PMS custom pantograph on the front, and PMS 287 blue for the state seal (screened at 50 percent), and PMS 403 gray on the back. The contractor must design and provide pantograph choices containing a hidden word "VOID" in erasable ink that appears when machine copied, include this cost in the bid price, and handle all artwork positioning of the pantograph, state seal, and border. Certificates must be shrink wrapped in packages of 500 with chipboard protection, with uniform cartons labeled with agency order number and form number but no form title, weighing no more than 40 pounds each. The contractor must submit material samples for approval prior to production, provide a color proof plus an additional alignment proof, and certify upon completion that all materials have been secured and waste materials properly destroyed to prevent unauthorized use.
