About this file

This is Addendum #6 to the Employee Assistance Program (EAP) Request for Proposal (RFP) issued by the Oregon Educational and Employer Benefit Board (OEBB) under OregonBuys Bid No. S-44300-00008777. The addendum amends the RFP schedule and minimum requirements for EAP service providers. Key schedule updates include a pre-proposal conference on December 14, 2023, questions due by January 2, 2024, answers issued approximately January 10, 2024, an RFP protest period ending January 17, 2024, presentations or interviews scheduled for March 18-22, 2024, notice of intent to award issued approximately April 2, 2024, and an award protest period ending April 9, 2024. The addendum specifies that EAP services must be available to employees not enrolled in a medical plan, applicable dependents including spouses and children, household members who are not dependents such as parents, and at both physical locations and virtually. Proposers must submit complete and responsive proposals addressing all applicable questions and required documents, with mandatory submissions including responses to Attachments A, B, C, F, G, and H, completion of the Final Submission Form (Attachment I), and a Disclosure Exemption Affidavit (Attachment D) by February 8, 2024.

The addendum emphasizes that OEBB requires proposals to be complete and compliant with all RFP specifications, with any missing information potentially resulting in proposal disqualification. Proposers must certify that they prepared their responses independently without collusion or fraud and must complete all state-required certifications in Part C of the Final Submission Form. Word limitations on certain responses, as specified in ProposalTech, are considered RFP requirements. The addendum does not address pricing terms, set-asides, incumbent vendor information, or funding sources, as these details would be contained in the main RFP documents and related attachments.

By completing Attachment I, Final Submission Form, Proposers will also agree to a number of representations and warranties described in Part B, such as agreeing that Proposer prepared its response to this RFP independently from all other Proposers, unless otherwise permitted by this RFP concerning subcontractors and collaborators, and without collusion, fraud, or other dishonesty.

In addition, Attachment I, Final Submission Form, also requires Proposers to complete Part C, which contains a number of state-required certifications.
