About this file
This is Addendum No. 4 to Request for Bid No. RFB-109-26 issued by the Commonwealth of Kentucky Department for Facilities and Support Services Division of Engineering and Contract Administration for roof replacement at the Eastern Kentucky Veterans Center in Perry County, Kentucky. The project involves the installation of a two-ply styrene-butadiene-styrene (SBS) modified bituminous membrane roofing system over existing roof membranes across multiple roof areas designated A through Q, with total roof areas ranging from 33 square feet to 23,211 square feet depending on the section. The addendum, dated December 4, 2025, makes modifications to specifications and drawings by omitting the original Specification Section 075216 and replacing it with an updated version, and by omitting Sheets A.1 through A.5 and replacing them with revised sheets. The project includes installation of 1/2-inch cover board insulation, base and cap sheet installation, metal flashing, gutters, downspouts, walkway pads, roof drains, and various penetration details. Acceptable roofing system manufacturers include Johns Manville, Siplast, Soprema Inc., and Elevate (formerly Firestone Building Products).
The contractor must comply with a 20-year manufacturer warranty without dollar limitation and non-prorated coverage for wind speeds up to 72 miles per hour, plus a two-year installer warranty from the date of substantial completion. The specification requires that the roofing system installer be approved and certified by the roofing membrane manufacturer and eligible to receive the manufacturer's special warranty. Installation must proceed in the presence of the manufacturer's technical representative and include preliminary and preinstallation roofing conferences with the owner, architect, installer, and manufacturer. Final inspection must be arranged with the manufacturer's technical personnel within 48 hours notice to the architect and owner. Weather limitations apply, with work proceeding only when conditions permit installation in accordance with manufacturer specifications and warranty requirements. The project includes detailed submittals for product data, shop drawings, samples for verification, manufacturer certificates, and qualification statements prior to installation.
