About this file

This is Addendum No. 2 to a Request for Proposal (RFP HS-24-16) issued by San Bernardino County Human Services for Child and Family Team Facilitator (CFTM) services. The addendum provides answers to bidder questions submitted by the December 29, 2025 deadline. In 2025, San Bernardino County completed 3,752 CFTMs for class children, of which 996 were initial CFTMs conducted by internal Jurisdiction/Disposition units. Excluding initial CFTMs, 2,756 ongoing CFTMs were completed by outside providers. Based on current caseload projections of 6,965 active class children, the county estimates a need for approximately 7,710 CFTMs annually to ensure all class children receive the required two CFTMs per year. CFTMs are conducted both in-person and via telehealth in nearly equal proportions, with sessions lasting between 1.5 to 2 hours. Approximately 70% of CFTM referrals involved sibling sets with an average of 3.9 children per referral. Language demographics show that 93% of active class children are English-speaking, 6% speak Spanish, and less than 1% speak other threshold languages.

The compensation structure provides a flat fee of $250 per CFTM regardless of the number of youths served in a referral or sibling set, with reimbursement issued on a fee-for-service basis for each completed CFTM referral. Contract staff are required to administer the Child and Adolescent Needs and Strengths (CANS) assessment, which is a comprehensive evaluation tool covering behavioral and emotional domains, cultural factors, life functioning, risk behaviors, caregiver resources and needs, and potentially traumatic experiences. The county was unable to separate utilization data between in-county and out-of-county youth, and information regarding children placed outside the county was not available at the time of the addendum. The county does not track CFTMs requested specifically by biological parents, though all completed referrals are eligible for reimbursement.

ADDENDUM NO. 2

Answers to questions submitted by the December 29, 2025 deadline:

1. Question: How many CFTMs were completed in the last 12 mos and how many are anticipated in the Next 12 mos?

Answer: In 2025, there were 3,752 Children and Family Team Facilitators (CFTMs) for class children, including initial CFTMs. There were 996 initial CFTMs, which are done by the Jurisdiction/Disposition (JD) units/new facilitation units in the future. If you exclude initial CFTMs from the 2025 count, then you have 2,756 CFTMs that are ongoing for class children that should have been done by an outside provider.

Based on numbers from 2025 and the criteria that each class children will have 2 CFTMs in the year, it is estimated there would need to be 7,710 CFTMS completed for all class members to be up to date with their CFTMs. This is estimated by doubling the number of class children active in 2025 (6,965x 2 = 13,930) and dividing it by the average number of children at each CFTM (13,930/1.6= 8,706) and finally subtracting the number of initial CFTMs done in 2025 (8,706-996= 7,710).

2. Question: How many CFTMs were held in person versus telehealth for in-county and out-of-county youth?

Answer: We are unable to separate In and Out of County Youth, but when we looked at Youth with a current CFTM in Safe Measures in December 2025, it was almost an exact 50/50 split between telehealth and in person. That would include subclass CFTMs. The information is not available for children placed outside the county.

3. Question: What is the average length of time for the CFTM?

Answer: CFTM take from 1 and ½ hours to 2 hours.

4. Question: Can you confirm if the $250 per CFTM rate is per youth or for an entire sibling set?

Answer: The fee per CFTM is $250, there is no difference in rate based on the number of youths.

5. Question: How many CFTM referrals were for sibling sets? Average number of youth per sibling set referred?

Answer: Of the 3,752 Class CFTMS completed in 2025 (including Initial CFTMs), 70% had multiple children. If there were multiple children, the average count was approximately 4 (3.9) children.

6. Question: How often did you complete multiple CFTMs for one child to meet the bio parent requests? Are you reimbursed for each CFTM?

Answer: There is no method of tracking CFTM requested by biological parent. Yes, reimbursed for each completed CFTM Referral as per the fee-for-service schedule.

7. Question: In the past 12 months, how many CFTMs were held in English, Spanish, and other threshold languages?

Answer: For the active class children in 2025, it was found that only 6% spoke Spanish as a primary or secondary language. Less than 1% of class children spoke a primary or secondary language other than English or Spanish.

8. Question: Is the CANS expected to be administered by the CFT Contract staff?

Answer: Yes, please refer to section V. Paragraph B. PROJECT DESCRIPTION in the RFP.

9. Question: Could you clarify what the CANS “Report” is (RFP PDF page 35 & 51/60)?

Answer: California Integrated Practice -Child and Adolescent Needs and Strengths is an assessment report for Children and Adolescent Needs and Strengths to be completed per RFP pages 12 – 15 that includes domains ranging from Behavioral/Emotional, Cultural Factors, Life Functioning Domain, Risk Behaviors, Caregiver Resources and Needs, and Potentially Traumatic Experiences.
