About this file

This is an addendum to a Request for Proposal issued by San Bernardino County Human Services for Child and Family Team Facilitators (RFP HS 24-16, ePro Bid #HSSA26-HSSA-5566). The addendum amends the definitions and project description sections to clarify contractor responsibilities in facilitating Children and Family Team Meetings (CFTMs) in compliance with California Department of Social Services (CDSS) guidelines and Welfare and Institutions Code requirements. The contractor must accept and process CFTM referrals, complete meetings within thirty calendar days of referral receipt, locate accessible facilities throughout San Bernardino County, and conduct pre-meeting preparation discussions with families. Facilitators must hold a minimum of two years of experience in family and youth meeting facilitation, and staff must receive training in IP-CANS assessments, cultural competence, trauma-informed care, and other CDSS-mandated topics. The contractor is responsible for coordinating with County Child and Family Services staff, Department of Behavioral Health, Probation, and other stakeholders to ensure coordinated care and case planning for children and youth in the child welfare system. CFTMs may be conducted via videoconference or teleconference during extraordinary circumstances with prior County approval.

The contractor must maintain confidentiality in accordance with Welfare and Institutions Code Section 832 and HIPAA regulations, obtain written authorization from participants for information sharing, and complete Summary and Action Plans in CDSS-required formats. Spanish-language CFTMs must be provided for monolingual Spanish-speaking families as needed. The contractor must use only County-approved forms and training materials and obtain written County approval in advance for any additional forms or presentation materials. Monthly reports must be submitted to the County in an approved format, listing active clients with dates of meetings, child or youth identifying information, and case numbers. The contractor is required to notify the County immediately if staffing levels interfere with service delivery or if disciplinary or suspension actions are taken against professional staff by the governing board.

RFP HS 24-16

ePro Bid #HSSA26-HSSA-5566

ADDENDUM NO. 1

Amend SECTION III. DEFINITIONS AND REFERENCE DOCUMENTS to read as follows:

III. DEFINITIONS AND REFERENCE DOCUMENTS

The following Definitions have been added or modified:

California Department of Social Services (CDSS): Provides oversight of California’s Child Welfare and Foster Care system, Welfare-to-Work programs, Disabled and Adult programs, and licensed community care facilities. The mission of CDSS is to serve, aid, and protect needy and vulnerable children and adults in ways that strengthen and preserve families, encourage personal responsibility, and foster independence.

CFTM Referral: A referral form used to request a CFTM.

Integrated Practice Child and Adolescent Needs and Strengths (IP-CANS): A multi-purpose tool that supports decision-making, including level of care and service planning, which allows for the monitoring and outcome of services. When used as part of the CFT process, the IP-CANS Assessment can help guide conversations among CFT members about the well-being of children and youth, identify their strengths and needs, inform and support care coordination, aid in case planning activities, and inform decisions about placement.

Welfare and Institutions Code (WIC): The set of laws in California that govern various social services and public welfare programs. The range of issues addressed in this code includes services relating to welfare, dependent children, mental health, and other areas affecting vulnerable populations.
