About this file
This is Addendum No. 2 to the Request for Proposal (RFP) for the Transitional Housing Program-Plus (THP-Plus) issued by San Bernardino County Human Services & Behavioral Health (RFP No. HS 24-07, ePro Bid #HSSA26-HSSA-6157). The addendum provides answers to 28 questions submitted by prospective bidders regarding program requirements, evaluation criteria, personnel requirements, licensing, data collection, proposal submission procedures, and financial documentation. Key dates include a non-mandatory proposal conference on November 18, 2025, a question submission deadline of November 25, 2025, a revised proposal submission deadline of December 8, 2025 at 4:00 p.m. PST, and a tentative contract award date of February 24, 2026 with a contract start date of March 1, 2026. The county will award one or multiple contracts based on proposals that best meet county needs. Proposers may submit proposals either electronically through the ePro system or via hard copy with original signatures to the San Bernardino County Human Services office at 150 South Lena Road, San Bernardino, CA 92415-0515.
Compensation is structured at $2,900 per youth per month for the Scattered Site Model and $3,300 per youth per month for Parenting Placement. Current contracts accommodate up to 60 slots per month, with the exact number of beds to be determined based on the number of vendors and contract amounts awarded. Proposers are not required to be registered in San Bernardino County but must meet all RFP requirements, including obtaining local fire clearance and county certification. All personnel must be in place prior to service delivery, and all insurance requirements must be fulfilled before contract commencement. The addendum clarifies that non-profit agencies need not hold state licensure as THP-Plus providers if certified by the county, establishes a case manager-to-youth ratio of 1:8-12, and specifies that proposers are responsible for delineating their service area and anticipated number of participants. Incentives for post-discharge follow-up participation are not reimbursable and remain at the contractor's discretion.

San Bernardino County Human Services & Behavioral Health Request for Proposal Transitional Housing Program-Plus RFP No. HS 24-07 ePro Bid #HSSA26-HSSA-6157
REQUEST FOR PROPOSAL FOR
Transitional Housing Program – Plus (THP-Plus)
RFP HS 24-07
ePro Bid #HSSA26-HSSA-6157
ADDENDUM NO. 2
Answers to questions submitted by the November 25, 2025 deadline:
1. Question: For Clarification purposes, the Evaluation Criteria is weighted as indicated in the RFP.
Answer: See RFP Page 34 for evaluation criteria.
2. Question: Does the non-profit have to be registered in San Bernardino County?
Answer: No, but the agency must meet requirements as stipulated in the RFP.
3. Question: Can the insurance requirements be fulfilled before the start of the contract or must it be done before the proposal is submitted?
Answer: All insurance requirements must be met before beginning services under the contract.
4. Question: Can my LLC be the owner of the home and do a Master Lease with On The Rise who then sub-leases to the NMD?
Answer: Please refer to Section V. Scope of Work, Paragraph F - Housing Requirements, in the RFP. Additionally, the functions of property management and service provider shall not be blended. The program plan shall clearly define the roles and responsibilities of each part of the organization (MPP 30-920(o)). This is especially relevant if the owner of the LLC is the proposer.
5. Question: Must all personnel be in place upon submission of proposal? Or if we are awarded a contract, must everything be in place by the start of the contract?
Answer: All personnel would need to be in place prior to providing services under the contract.
6. Question: Must we obtain a license with the state?
Answer: Agencies must meet any licensing requirements. THP-Plus providers are not subject to licensure by the California Department of Social Services, as long as they are certified by the county to provide transitional housing and supportive services, as needed, and have obtained a local fire clearance
7. Question: Is there anywhere in the proposal where we should specify a number of beds/participants or service area?
Answer: Proposer is responsible for delineating the services they are able to provide including the number of participants they anticipate serving and the service area(s).
8. Question: Who do I follow up with regarding the answer to my question?
Answer: All answers to Proposer’s questions will be posted as an Addendum in ePro.
9. Question: Is the County looking to award multiple contracts to multiple providers or one provider for the County?
Answer: The County will award a contract or multiple contracts based on the proposals that best meet the needs of the County.
10. Question: Is the County looking to award multiple contracts to multiple providers or one provider for the County?
Answer: See Answer to Question #9.
11. Question: What is the total amount per youth?
Answer: Scattered Site Model $2,900 per youth per month; Parenting Placement $3,300 per youth per month.
12. Question: What is the total number of beds?
Answer: Number of beds will be determined based on number of vendors and contract amount. Current contracts accommodate up to 60 slots per month.
13. Question: Page 29 of the RFP Section H.iii mentions data collected after participants have left the program, up to a year, will be furnished to the County ILP Coordinator in the form of the John Burton Foundation six (6) month and twelve (12) month follow-up reports. How is this to be collected if John Burton is no longer available?
Answer: Contractor is still responsible for data collection and must provide the information to CFS upon request.
14. Question: Can you clarify what is covered or qualifies as an incentive?
Answer: Incentives are generally cash-equivalents, i.e., gift cards.
15. Question: Page 29 of the RFP Section H.iii states we may provide incentives after discharge. What could these incentives be and will they be reimbursed?
Answer: See page 29 of 67, Project Description, Section B, paragraph 3, item H(iii): For a minimum of one (1) year following discharge from the program, the Contractor must complete follow-up assessments and outcome evaluations every six (6) months. Contractor may offer incentives to the young adults to encourage their participation. This data will be furnished to the County ILP Coordinator in the form of the John Burton Foundation six (6) month and twelve (12) month follow-up reports. See Answer to Question #14. Incentives are not reimbursable, these incentives are at the discretion of the contractor.
16. Question: Page 30, Section VI Proposal Submission, Paragraph B.1 asks for an original copy which may be bound and another copy unbound. Can you clarify what you mean by bound and unbound? Can they both be placed in a 3-ring binder?
Answer: Bound refers to how the proposal is assembled, such as stapled or placed in a 3-ring binder.
17. Question: Page 35 of 62, SectionVI. Proposal Submission, Paragraph 7 - Financials: The County is requesting our Agency "Annual Report" and we "must also include two (2) most recent financial statements." Can you explain the difference between these two reports and what information is anticipated as part of the Annual Report?
Answer: “Annual Report” is referring to an optional comprehensive report that provides an overview of the entity’s financial performance, operations, and overall condition.
“Independently audited financial statements” is referring to financial statements that are audited by a Certified Public Accountant (CPA). If your entity does not have this, unaudited financial statements are acceptable along with an explanation of why you do not have audited financial statements.
18. Question: When do we expect to receive answers for the questions submitted, considering the holiday closure?
Answer: The answers to questions have been posted as an Addendum in ePro.
19. Question: Page 17 of 67: Section V. Scope of Work, Paragraph B, Item K iii. Please clarify this statement: Have a ratio of Case Managers working with young adults and pregnant and parenting young adults of one (1) Manager to eight (8) to twelve (12) young adults.
Answer: We are asking for a Case Manager to young adult ratio of 1:8–12. Each Case Manager will oversee between 8 to 12 young adults.
20. Question: Provider Plan – is there a template for the provider plan?
Answer: Please see Attachment E.
21. Question: Section IV – Proposal Submission, Item B.1 states that hard copies are required and must also be submitted electronically. Although County representatives indicated that both submissions are not necessary, would it be possible to receive clarification on this point in writing? Are hard copy submittals required?
Answer: County will accept Electronic or Hard copy submission of proposals.
22. Question: Page 22 of 67: Section V. Scope of Work, Paragraph B, Item 3A - Referral procedure: Requirement does not match current practice. Are we to keep the practice outlined in the RFP or will this be updated?
Answer: The process will be as outlined in the RFP. As noted on page 22 of 67, under Referral Procedure section, the County and Contractor will follow the requirement as currently stated.
23. Question: Page 23 of 67, Section V. Scope of Work, Paragraph 3Ci. states: Review the young adult’s application, including review of up to two (2) references. Many youth in need will lack references. This is not a requirement of the current contract. Is this a new practice to be implemented?
Answer: The process will be as outlined in the RFP. As noted on page 23 of 67, under Referral and Screening Process section, the item requires reviewing references as part of the application review process and does not necessarily require that there be two references. This item sets a maximum of references to be reviewed, not a minimum.
24. Question: Page 23 of 67, Section ii.6 states: Development of the Transitional Independent Living Program (TILP) must be part of the Intake Assessment. Can there be more time for assessment to develop the TILP following placement?
Answer: Please refer to the intake procedure as indicated on page 23 of the RFP, specifically section iii for timeline requirements.
25. Question: Definition of “Three (3) Years of Experience Providing Similar Services” — Does this mean specifically operating a THP Plus or THPP program, or would experience in foster care, STRTP, or Full Service Partnership programs be acceptable?
Answer: Similar services generally refer to experience operating programs that serve comparable youth and provide related services as described in the RFP. Experience operating a THP-Plus or THPP program would best qualify, provided all other requirements are satisfied. The County may consider experience in other transitional housing programs.
26. Question: Timeline for Posted Responses — The RFP indicates that written Q&A will be posted to ePro within two weeks following the question deadline. Can you confirm the anticipated date?
Answer: The Q&A has been to ePro.
27. Question: Time Between Q&A Release and Proposal Deadline — Given the short time between the Q&A posting and the December 4, 2025, 4:00 p.m. PST deadline, are there any updates or considerations regarding the timeline?
Answer: The deadline for submission of proposals has been extended to Monday, December 8, 2025, 4:00 p.m. PST.
28. Question: Total amount to be paid per youth?
Answer: See Answer to Question #11 - Scattered Site Model $2,900 per youth per month; Parenting Placement $3,300 per youth per month
