About this file

This Addendum No. 1 clarifies key terms for the Pharmaceutical Supply Services annual contract (RFB No. 23-0013) issued by the City of Columbus, Georgia's Finance Department Purchasing Division for Columbus Fire and Emergency Medical Services (CFEMS). The City intends to award the contract to a single vendor and has confirmed it possesses the necessary regulatory credentials to receive pharmaceutical items, specifically holding a valid DEA registration certificate (Form DEA-223) for controlled substances through April 30, 2025. The facility receiving shipments is identified as Columbus Fire and EMS Logistics located at 1905 3rd Ave, Columbus, GA 31901, with the current medical director listed on the DEA registration.

This addendum supersedes or modifies provisions contained in the original RFB No. 23-0013 solicitation documents. Vendors must acknowledge receipt of this addendum using the Addenda Acknowledgement form provided in the solicitation package, as failure to do so may result in the bid being deemed non-responsive and ineligible for award consideration. The City reserves the right to add related pharmaceutical items during the contract term and will obtain quotes from the contracted vendor as needed.
