About this file
This document is Addendum No. 01 issued by the Louisiana Office of State Procurement for the Invitation to Bid (ITB) related to RFx Number 3000024780 for Rebid Lab Supplies for the Department of Transportation and Development (DOTD). The addendum modifies a specific line item, changing Line 3 of the RFx from "One (1) - #10" to "One (1) - #8", and is dated May 07, 2025. The bid is scheduled to open at 10:00 AM CT on 05/20/2025, and the addendum provides instructions for bidders who have already submitted their bids or need to make revisions due to this change.
The addendum includes provisions for acknowledging receipt of the document, with options for bidders to return an acknowledgement form by mail, hand delivery, courier, or fax. Bidders who have already submitted their bid and do not need to revise it can simply acknowledge the addendum, while those requiring revisions must clearly indicate changes and submit them prior to the bid opening. The state reserves the right to request a completed acknowledgement at any time, and failure to execute an acknowledgement does not relieve the bidder from complying with the terms of their bid. Contact information for follow-up is provided for Dorian Allen from the Office of State Procurement.
