About this file
This is an addendum to the request for bids issued by Ozaukee County for construction services at the Lasata Senior Living Campus in Cedarburg, Wisconsin. Addendum F, issued December 4, 2025, modifies the original contract documents and five previous addenda by revising the Instructions to Bidders, updating the Work Schedule acknowledgement deadline to December 12, 2025, and providing detailed scope clarifications for Contract #15 (Food and Service Equipment). The project involves construction of a new Community Based Residential Facility with four remaining contract packages available for bidding: Contract #5 (Roofing/Siding, with siding awarded), Contract #6 (Steel Studs & Drywall Assemblies/Insulation), Contract #10 (Fire Protection Systems), and Contract #15 (Food and Service Equipment). Sealed bids must be submitted to the Ozaukee County Administrator's Office at 121 W Main Street, Port Washington, Wisconsin by 2:00 p.m. CST on Tuesday, December 9, 2025, with public bid opening occurring at the same time and location. An optional pre-bid site visit was scheduled for July 31, 2025 at 2:00 p.m. CST at W76N677 Wauwatosa Road, Cedarburg, Wisconsin. The revised milestone schedule attached to this addendum establishes project completion by July 17, 2026, with kitchen equipment installation occurring between March 10 and April 20, 2026.
Bid security in the amount of five percent of the bid amount must be submitted with each proposal in the form of a bid bond or certified check, with successful bidders required to furnish 100 percent performance and payment bonds prior to commencing work. The addendum addresses four specific RFI clarifications regarding division specifications, kitchen equipment demonstration plans, onsite storage for existing equipment, and updated project scheduling. Existing food service equipment will be temporarily relocated to a vacant unit during construction to maintain uninterrupted kitchen operations. Bidders are responsible for examining all contract documents, acknowledging receipt of all addenda, and complying with comprehensive insurance requirements including general liability coverage of $2,000,000 per project, automobile liability of $1,000,000, and umbrella liability of $2,000,000 per occurrence. The project is not subject to prevailing wage requirements, and Ozaukee County is tax-exempt. All contract documents incorporate AIA 132CMa-2019 agreement forms and require bidders to acknowledge all issued addenda or risk bid rejection.
