About this file

This document is an Addendum D for the Lasata Senior Living Campus New CBRF (Community Based Residential Facility) project located in Cedarburg, Wisconsin, issued by Dimension IV - Madison, LLC. The addendum modifies the original Contract Documents dated 5/9/2025, with previous addenda issued on 6/30/2025, 7/16/2025, and 8/5/2025. The bid submission deadline is set for Tuesday, August 19th, 2025 at 2:00 PM CST at the Ozaukee County Administrator's Office. The addendum includes changes to the Project Manual, specifically updating sections related to fiberglass windows, plastic laminate cabinets, and various drawing revisions for the new senior living facility.

The addendum provides technical specifications and clarifications for the project, including window performance requirements (U-factor of 0.29 and SHGC of 0.36), window hardware details, and interior finishes. It addresses multiple Request for Information (RFI) questions regarding window types, glazing, hardware, casework, and electrical systems. The changes impact various aspects of the project, such as window limiters, cabinet specifications, and electrical infrastructure. The document indicates that the electrical contractor's scope includes providing infrastructure and cabling for access control, security cameras, and nurse call systems, with some coordination required with the owner's vendor.

RFI QUESTIONS / CLARIFICATIONS:

Design team responses in red.

1. Clarification : Addendum A and B are included as part of the issued for Bid drawing set.

2. Fiberglass Windows - Please confirm all fiberglass units should be tempered due to cost – not all fiberglass window glazing needs to be tempered, Please see Addendum D - A6.3.

3. Fiberglass Windows - Please confirm limiters for hungs. Spec shows limiters for casements. – please see Addendum D, a note has been added on A1.0 indicating which windows require a limiter. Limiter is deleted from casement section and added to the hung window section.

4. Plam Countertop Casework and Shelving Spec Section - It in indicated on Spec Manual that Fabricator must be a certified participant in AWI, some of the bidders are asking, particularly those participating on the general trades if this certification is in strict compliance or rather than comply with AWS/AWI Standards. – Please see Addendum D.

line is deleted from spec section.

5. Millwork - Kitchen: Elevation 1/A7.0 calls for PL-1 upper cabinets, but the Room Finish

Schedule on A9.0 call for CAB-1 upper cabinets. Which is correct? Upper cabinets should be CAB-1, Please see Addendum D

6. Millwork - Medicine: Elevations 17,18/A7.1 calls for PL-1 countertop, but both the Room

Finish Schedule and Finish List on A9.0 call for PL-2 at the countertop. Which is correct?

Countertop should be PL-2, Please see Addendum D

7. Millwork - Pantry: Elevation 4/A7.0 calls for PL-1 countertop, but both the Room Finish

Schedule and Finish List on A9.0 call for PL-2 at the countertop. Which is correct?

Countertop should be PL-2, Please see Addendum D

8. Millwork - Laundry: Elevation 5/A7.0 calls for PL-1 countertop, but both the Room

Finish Schedule and Finish List on A9.0 call for PL-2 at the countertop. Which is correct?

Countertop should be PL-2, Please see Addendum D

9. Millwork - Spa & Toilet: Elevations 6,11,13/A7.1 calls for CAB-1 wood apron/skirt, but both the Room Finish Schedule on A9.0 calls for PL-1. Which is correct? Lav Apron/Skirt should be PL-1, Please see Addendum D

10. Millwork - Do you have a spec for the open shelving brackets (in the Pantry & the

Laundry), or will an allowance for KV 0208 series “Ultimate L-Brackets” be acceptable?

An allowance for brackets is acceptable.

11. Millwork - Is rail R1 in the custom millwork scope? Rail R1 per 1/A6.1 refers says to see

6/G0.2 for details. The handrail at 1/A6.1 shows a standard round handrail with brackets, but 6/G0.2 shows an accessible contoured handrail. Which is correct? Rail R1 should be standard round handrail with brackets. The G0.2 sheet is indicating the accessibility requirements of the railing, not the shape of the rail.

12. Access Control – Could you please confirm whether the electrical contractor’s scope is limited to providing infrastructure and cabling only, per the device locations indicated on the drawings? Correct. Additionally, what specific type(s) of cable should be provided?

00901 - 4

Or will the final cable specification and routing be determined by the owner’s vendor?

This should be confirmed with the owners vendor.

13. Nurse Call System - While the drawings indicate nurse call device locations, there is no accompanying specification, general notes, or direction. Can you clarify the intended scope? Should the electrical contractor provide conduit only, or also furnish and install cabling? If so, what system and cable type are expected? The intent is to extend into the existing system located in Maintenance Shop 007 of the Crossing Building. This would include conduit, wire, devices and programming.

14. Security Cameras (CCTV) - Please confirm the electrical contractor’s responsibilities are just to install conduits and CAT6A Jack and Cable? Correct

15. Ceiling Plenum Designation - Is the space above the suspended drop ceiling considered a plenum throughout the project? There are no plenum spaces in the new CBRF building.

The area above the kitchen work in the Crossings building has a current return air plenum. Note #5 on Sheets E2.0B and E2.0C is the only reference to plenum rating.

Please confirm whether this applies project-wide and the reasoning behind the plenum classification (e.g., return air path or specific code requirement). Note #5 will be removed from sheets E2.0B and E2.0C. It will then be added to sheet E1.1. (see Addendum C)

16. Acceptability of NM Cable - Lastly, is the use of NM (non-metallic) cable permissible in this project? Or should branch wiring be run in MC, EMT, or other approved wiring methods, particularly if the ceiling space is indeed plenum-rated? NM is to not be used on the project. At a minimum use EMT.

17. Acceptability of MC Cable - per specifications the electrical components must be compliant with the National Electrical Code, which they were saying that if at minimum

EMT will be used, it will be more expensive and overdesigned. They asking if the NM

Cable is not an option, can an MC Cable be considered? keep EMT as originally designed and specified, but the contractor may present MC as a value engineering item.

18. Referring to Sheet A9,0 – Room Finish Schedule, the table is lacking the typical unit finish schedule. Please see Unit Finishes in upper right corner of A9.0.

19. The specs call for double hung and the window schedule in the drawings show single hung. Double-Hung windows will be used. See Addendum D – A6.3 revised.

20. The specs call for U-Value of 0.30 and SHGC of 0.40 and the drawings call for U-Value of

0.29 and SHGC of 0.36. U 0.29 and SHGC of 0.36 should be used. See revised spec section Addendum D.
