About this file

This document is an Addendum to the Request for Quotation (ARFQ) JWH25*1 for Physical Therapy Services to the residents of Jackie Withrow Hospital. The Addendum provides answers to several questions received regarding the procurement, including that there is no local vendor preference, that only one award will be made to the lowest cost responsive bidder, and that this is not a new initiative but a continuation of services currently provided by Elizabeth Wallen PT at a rate of $75 per hour. The vendor will not be required to furnish equipment, supplies, or materials, as the facility will purchase any needed items. The current annual spending is approximately $30,000, but the agency cannot disclose the estimated budget. The Addendum also clarifies that the service level is based on 35 hours per month of therapy services, rather than the 30 hours per month originally specified.

Regarding pricing, the Addendum notes that the vendor can use any number of therapists as long as the specifications are met. The contract is for Physical Therapy services to the residents of Jackie Withrow Hospital, which is located in Kanawha County and Charleston City, West Virginia.

ARFQ JWH25*1 Addendum 1: Attachment 1 The following questions were received regarding ARFQ JWH25*1 (Physical Therapy Services to the residents of Jackie Withrow Hospital.)

Question #1: Is there a local vendor preference on this bid?

Answer: No, as long as the vendor can provide a Physical Therapist (s) to meet the specifications is doesn’t have to be local and there is no local preference.

Question #2: What is the anticipated number of awards for this project?

Answer: 1, this will be awarded to the vendor with the lowest total cost meeting all specifications.

Question #3: Is this a new initiative? If not who are the current vendors and please share their pricing file.

Answer: No, this is not a new initiative, our current provider is Elizabeth Wallen PT and we currently pay this contract at an hourly rate of $75 per hour.

Question #4: Is the vendor required to furnish equipment, supplies, or materials?

Answer: No, the vendor will make recommendations to the facility for any specific resident need items, and the facility will purchase these items.

Question #5: What are the historical annual spending volumes in this project?

Answer: Currently Approximately $30,000.00 per year

Question #6: What is the estimated budget for this project?

Answer: We can’t disclose our estimated budget.

Question #7: The solicitation establishes Physical Therapy Treatment for and undetermined number of the 55 to 65 inpatients monthly. This service level includes new patient assesments, treatment planning, therapy documentation, and annual chart reviews and reporting that will require extensive time to perform thoroughly and effectively. The solicitation does not identify the number of hours per month that will be budgeted to provide these service levels, therefore it is difficult to respond to the pricing sheet requirement for a total annual cost for the services. Please advise how many hours of treatment and documentation is anticipated annually and how many therapists will be allowed to perform the contract services.

Answer: Refer to section 4.1.1.13 in the specifications, as these services will be based on 30 hours per month, but we are changing this to 35 hours per month. It doesn’t matter how many therapists the vendor chooses to provide as long as the specifications are met.

4.1.1.13 currently reads “Physical Therapist must coordinate with Recreation Supervisor and Director of Nursing in which time must be worked. This will be during normal business hours (Monday thru Friday 8a to 4p excluding any legal holidays.) Facility will pay the therapist a monthly rate in arrears of all services rendered. Monthly rate will be based on 30 hours.”

This is being changed to “Physical Therapist must coordinate with Recreation Supervisor and Director of Nursing in which time must be worked. This will be during normal business hours (Monday thru Friday excluding any legal holidays.) Facility will pay the therapist a monthly rate in arrears of all services rendered. Monthly rate will be based on 35 hours.”
