About this file

This is Addendum 2 to a Request for Bids issued by the City of Madison, Wisconsin Finance Department Purchasing Services for magnetic bus passes to be supplied to Metro Transit. The addendum corrects a posting error regarding the original Form D and requires bidders to acknowledge receipt on RFP Form B and the Receipt Forms and Submittal Checklist. The contract requires vendors to supply an estimated 1,790,000 total units across six product categories: 140,000 magnetic stripe bus passes (up to 20 types), 25,000 magnetic stripe UW passes (approximately 1 type with codabar), 25,000 magnetic stripe youth passes (approximately 3 types), 400,000 direct thermal plastic bus passes (up to 10 types), 1,200,000 transfers (1 type), and 10,000 MiFare smartcards (blank, no encoding, approximately 1 type). Bidders must specify delivery time in days after receipt of order.

Cost proposals must be submitted as all-inclusive, not-to-exceed, fixed fees covering all direct and indirect expenses including travel, telephone, and copying. All prices, rates, fees, and conditions shall remain fixed for the contract term and any renewals, with any increases or additions requiring written agreement by both parties. The form requires vendors to provide unit prices, extended prices, and projected price increases or decreases for years 2021 through 2024 for each product category, as well as any available quantity discounts. Metro Transit reserves the right to modify quantities based on actual pass sales and will only pay for passes actually ordered. Vendors must obtain written confirmation from Metro Transit regarding production quantities prior to manufacturing.
