Addendum 2 to Request for Proposal B15-2026: Beavercreek ES Asbestos Flooring Removal

This is Addendum 2 to an Invitation to Bid issued by Oregon City School District on May 14, 2026, for asbestos-containing flooring removal at Beavercreek Elementary School located at 21944 S Yeoman Road, Beavercreek, Oregon 97004. The project scope involves the removal of presumed and confirmed asbestos-containing material (ACM) flooring, including 2,000 square feet of presumed-ACM non-friable 9-inch floor tile and black mastic under carpet in Rooms 3 and 5, and 1,250 square feet of 2% asbestos non-friable 12-inch floor tile and black mastic in three classroom activity pods (Rooms 17A, 29A, and 32A). A mandatory pre-proposal meeting was conducted on Wednesday, May 13, 2026, at 2:30 PM at Beavercreek ES. The addendum clarifies contractor responsibilities, including contracting with an environmental consultant for air clearances, subcontracting an electrician for 220/480V panel installation for grinding machines, and ensuring all mastic removal is completed prior to polishing concrete floors. The district will remove all furniture, non-fixed casework, and personal items from affected classrooms prior to contractor arrival, with rooms to be cleaned to a dust-free condition upon completion, including casework, walls, and ceilings.

The asbestos survey, completed by Apex Companies, LLC on February 27, 2026, identified specific areas of ACM through laboratory analysis performed by Eurofins LabCor. Contractors may use low volatile organic compound (VOC) mastic removers or disc sanding methods at their discretion for means and methods. Referenced documents include the Beavercreek Asbestos Records Summary, the Limited Flooring Asbestos Survey Report, 2021 construction documents for reference, and a pre-bid attendee sign-in sheet. The work must comply with Oregon OSHA regulations (OAR 437-003-1926.1101) and Department of Environmental Quality requirements (OAR 340, Division 248) for asbestos abatement, with all work performed by licensed and trained asbestos abatement contractors.

Addendum 2

Beavercreek ES Asbestos Flooring Removal Request for Proposal Number: B15-2026 Addendum Issued: May 14, 2026

ADDITIONAL / REVISED INFORMATION

The following information and documents / attachments listed below are to be included in the bid documents.

BIDDER QUESTIONS

1. Q: Who is paying for the air clearances? School or us? Any specific company to be used for clearance/air monitoring?

a. Contractor is to contract with an environmental consultant to perform clearances.

2. Q: Will 220V power be provided to power the grinding machines?

a. Contractor shall subcontract an electrician to install pig tail in 220/480V panel for power to the grinding machines.

3. Is it safe to assume that VCT is under all carpeted areas?

a. The full report on asbestos containing materials is attached to this addendum. No assumptions are required for these areas.

4. Is solvent an option? Would we be able to use a low VOC mastic remover for the flooring mastic or do you have a preference?

a. The use of low VOC mastic remover or disc sanding is acceptable and up to the contractor for means and methods of removal.

5. Do you have testing results (Survey)?

a. Yes. A survey of Beavercreek ES and the specific flooring in this project have been included in this addendum.

6. Will the teachers be removing all of their personal items attached to the walls, items hanging from the ceilings and items attached to case work to remain?

a. The rooms will have all contents removed from the floors by movers. The teachers will have any items that are not flat against the wall removed from the walls.

Ceilings will be free from anything hanging except for permanent fixtures.

Teachers will take home all personal belongings. Items will stay in closets, cabinets with doors.

7. Will all casework that remains upon the abatement companies' arrival be abated around or will some case work need to be moved by the abatement contractor?

a. All casework that remains will be fixed to the walls and not required to be removed. All non-fixed casework will be removed from the rooms by movers.

8. Will all furniture (and casework) that resides on top of the floors that will be abated within classrooms: 3, 5, 6, 17, 18, 19, 29, 30, 31, 32, 33 and 34 be removed prior to the abatement companies' arrival?

a. All furniture and non-fixed casework will be removed prior to the abatement companies’ arrival.

9. Additional grinding and polishing will need to occur after the 25 grit blades remove the mastic. Does the district acknowledge that the abatement contractor is not responsible for any subsequent grinding and polishing after the mastic has been removed via 25 grit blades?

a. The District expects that all mastic will be removed prior to the polishing of the concrete with 25 grit blades or at least 1 pass will be after the removal of all mastic if grinding is determined to be method of mastic removal.

10. Do you have testing results (Survey)?

a. Yes. A survey of Beavercreek ES and the specific flooring in this project have been included in this addendum.

Regarding: Limited Asbestos Survey

Beavercreek Elementary School – Limited Flooring Survey

Apex Project 25011371, Task 0006

Mr. Isaac, On February 20, 2026, Apex Companies, LLC (Apex), by and through its wholly owned subsidiary PBS Engineering and Environmental LLC, performed a limited pre-renovation asbestos survey of flooring in select locations of

Beavercreek Elementary School in Oregon City, Oregon. Apex is contracted to provide a comprehensive survey of the building, and Oregon City School District (the District) requested an expedited survey of flooring in select locations to facilitate abatement scheduled for Summer 2026. Apex has included conclusions from historic data in this survey report where noted.

The purpose of the survey was to locate, identify, and quantify accessible friable and non-friable asbestos-containing building materials for removal prior to renovation. The survey was limited to flooring and floor base in

Rooms 3, 5, 6, 17, 18, 19, 29, 30, 31, 32, 33, and 34.

The survey is also intended to satisfy Occupational Safety and Health Administration (OSHA) hazard communication requirements as well as requirements by the Department of Environmental Quality (DEQ) to perform an asbestos inspection prior to renovation or demolition activities under Oregon Administrative Rule

(OAR) 340-248-0270.

ASBESTOS SURVEY

The flooring areas were inspected by an Apex Asbestos Hazard Emergency Response Act (AHERA) accredited inspector to determine the presence, location, and approximate quantity of asbestos-containing materials (ACMs).

11 bulk samples of building materials, suspected of containing asbestos, were collected and submitted under chain of custody to Eurofins LabCor of Portland, Oregon, for polarized light microscopy (PLM) analysis. Historic data has also been incorporated into these conclusions and is attached as well. The following materials were found to contain asbestos:

mailto:Randy.Isaac@otak.com

Limited Asbestos Survey Report

At the time of this survey, floor tile and mastic was observed to be in good condition and non-friable, but removing carpet may cause the tiles to pop up, break, and become friable.

The following building materials are known by historic data to contain less than 1% asbestos. The Environmental

Protection Agency (EPA) and DEQ do not consider building materials that contain less than 1% asbestos to be

"asbestos-containing materials." These materials are included in the asbestos-containing materials section of this report for the sake of hazard communication since there are some OSHA restrictions associated with these materials.

• Joint compound associated with original construction gypsum wallboard walls and hard ceilings throughout the building. Joint compound is also incidentally adhered to materials attached to wallboard, such as covebase, backsplash laminate, wainscot, ceiling tiles, and corkboards, and will be disturbed if these components are removed. Specific to this project, removing covebase from gypsum wallboard walls will impact the joint compound.

Other sampled materials, such as carpet mastic, leveling compound, non-ACM floor tile/mastic in Room 6, and covebase/mastic were found to test non-ACM. Please refer to the attached map and asbestos bulk sample inventories for more sample details.

Asbestos Regulations

DEQ, Environmental Protection Agency (EPA), and OSHA regulations require proper removal and handling of ACM by licensed and trained asbestos abatement contractors prior to building renovations or demolition.

EPA, DEQ, and OSHA all define ACM as any material containing more than 1% asbestos. Although materials equal to or less than 1% are not considered by regulatory agencies to be an ACM, they still have some asbestos content, and Oregon OSHA has specific requirements for situations in which workers may encounter, disturb, or remove materials containing any level of asbestos. For the sake of hazard communication, these materials are included in the asbestos-containing materials section of this report.

In 1995, Oregon OSHA adopted 29 Code of Federal Regulations (CFR) Part 1926.1101 governing asbestos under

OAR 437-003-1926.1101. The regulation has made significant changes in work procedures and how asbestos materials are managed. OSHA believes that the single biggest risk of asbestos exposure is to workers who unknowingly or improperly disturb ACM. Hazard communication, training, personal protection, work practices, exposure monitoring, and recordkeeping are all major components of the regulation.

DEQ’s OAR 340, Division 248 also covers asbestos abatement requirements, removal notifications, licensing, and certifications for contractors.

For more information regarding the removal of asbestos-containing materials, please refer to the following:

1. Oregon Occupational Safety and Health Administration, OAR 437-003-1926.1101

2. Department of Environmental Quality, OAR-340, Division 248

This report is not suitable as a bid document or an asbestos abatement design. The purpose of this report is risk hazard communication only.

All samples were received in acceptable condition unless noted in the Report Comments portion in the body of the report. The results relate only to the samples as received and tested. The results include an inherent uncertainty of measurement associated with estimating percentages by polarized light microscopy. Measurement uncertainty data for sample results with >1% asbestos concentration can be provided when requested.

The report must not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST, or any agency of the federal government. The Company reserves the right to dispose of all samples after a period of thirty (30) days, according to all state and federal guidelines, unless otherwise specified.

All components not quantified as asbestos content and non-asbestos content are considered to be non-fibrous matrix components. Matrix components may include, but are not limited to, gypsum, paint, silicate minerals, vinyl, binder, calcium carbonate, tar, and foam.

Inhomogeneous samples are separated into homogeneous subsamples and analyzed individually. ND means no fibers were detected. When detected, the minimum detection and reporting limit is less than 1% unless point counting is performed. Floor tile samples may contain large amounts of interference material and it is recommended that the sample be analyzed by gravimetric point count analysis to lower the detection limit and to aid in asbestos identification.

C/O Kevin Moisan, CCM - Otak

Via email: Kevin.Moisan@otak.com

Regarding: Beavercreek Asbestos Records Summary

Beavercreek Elementary School

21944 S Yeoman Road

Beavercreek, Oregon

Mr. Moisan, Apex Companies, LLC, by and through its wholly owned subsidiary PBS Engineering and Environmental LLC (Apex), is in the process of completing a comprehensive survey of asbestos-containing materials (ACM) and lead-containing paint at Beavercreek Elementary School in Oregon City, Oregon. Oregon City School District has limited records of existing ACM and a map of known areas of ACM for this building, which are hosted on Verdant, the

District’s online recordkeeping platform. Apex has compiled that information for reference by the project design team here. This information has not been verified in the field by Apex, is not a substitute for a comprehensive survey, and does not meet requirements for performing a building survey prior to impacting ACM.

Some information presented here may be inaccurate or out of date, and some of these materials have been removed from the building or may be present in locations outside those listed here.

SUMMARY OF KNOWN POTENTIAL ASBESTOS-CONTAINING MATERIALS

The following is a summary of known asbestos-containing materials in the building and their general locations.

Material Location Note

Duct felt tape Attic areas at HVAC joints

Loose black roof debris Ceiling areas

Various vinyl floor tiles and black mastic Throughout, refer to figure

Exterior window glazing Kitchen windows

Sheet flooring Resource building bathroom Covered by newer material

Asbestos pipe insulation Interstitial wall and ceiling cavities

Hard fittings on fiberglass pipe insulation Interstitial wall and ceiling cavities

Asbestos Records Summary

August 19, 2025

Material Location Note

Mechanical isolation cloth West attic

Brown ceiling tile mastic Scattered throughout Less than 1% asbestos

Gypsum wallboard and joint compound Throughout main building Less than 1% asbestos

Brown covebase mastic

Throughout main building, often covered by newer mastic and vinyl base

Less than 1% asbestos

OTHER ASBESTOS-CONTAINING MATERIALS POTENTIALLY PRESENT IN THE BUILDING

Historic sampling and investigation in the building has often focused on finish materials such as flooring, walls, and ceilings. The following is a summary of categories of building materials that have largely not been surveyed by historic efforts, may be asbestos-containing, and should be considered by the design team. Note that many of these materials will likely not be asbestos-containing after sampling:

▪ Various vinyl floor bases and associated mastic.

▪ Various ceiling tiles and associated mastic.

▪ Chalkboards, corkboards, and associated mastic.

▪ Mechanical gaskets.

▪ Bricks and mortar.

▪ Ceramic tiles and mortar, grout, and mastic.

▪ Wire insulation.

▪ Fire doors.

▪ Roofing materials.

▪ Various window, door, and seam caulks.

▪ Exterior siding and panels.

▪ Exterior window glazing.

▪ Vapor barriers, tar paper, or asphalt waterproofing under brick, concrete, ceramic tiles, wood floors, or subflooring.

▪ Cement-asbestos drain pipes buried in slabs and soil.

LEAD-CONTAINING PAINT

Only very limited lead paint sampling has been conducted in the building. Given the various ages of construction in the building, it is a safe starting assumption that most painted surfaces outside of newer renovations have lead-containing paint.

Please feel free to contact me at 503.515.7489 or james.mastanduno@pbsusa.com with any questions or comments.

Sincerely, James Mastanduno

Project Manager
