About this file
This is Addendum 1 to a Request for Proposal issued by the City of Madison, Wisconsin Finance Department Purchasing Services for Drug and Alcohol Testing Services (RFP #11038-0-2022-AH). The addendum addresses fourteen questions submitted by bidders regarding the scope of services, current provider information, and operational requirements. The City requires drug and alcohol testing services including breath alcohol tests, urine drug collections, laboratory testing, observed collections, and random selection services. Testing must be conducted during normal hours (Monday-Friday, 6:00 AM - 6:00 PM CST/CDT) and after-hours (6:01 PM - 5:59 AM CST/CDT). The City prioritizes onsite collections at transit facilities for scheduled Federal Motor Carrier Safety Administration (FMCSA) and Federal Transit Administration (FTA) testing, with walk-in clinic services available as backup. Testing volumes are based on 2018 historical usage data and are not guaranteed for future years. The incumbent provider is Comprehensive Drug Testing, Inc. as the Third-Party Administrator (TPA), with Drug and Alcohol Testing, 24/7, LLC currently providing walk-in clinic services with hours of 8:00 AM - 4:30 PM Monday-Thursday and 8:00 AM - 3:30 PM Friday.
Pricing must be submitted as all-inclusive, not-to-exceed, fixed fees covering all direct and indirect expenses including travel, telephone, and copying costs. The cost proposal template requests pricing for eight service categories based on 2018 quantities: 128 Breath Alcohol Tests (Normal Hours), 21 Breath Alcohol Tests (After Hours), 6 Breath Alcohol Confirmation Tests, 369 Urine Drug Collections (Normal Hours), 48 Urine Drug Collections (After Hours), 386 Laboratory Testing services, 66 Observed Urine Collections, and 4 Random Selection Costs. Bidders must also provide pricing for Expert Witness Testimony and proposed annual increase percentages for contract years 2024-2027. The current contract ended because renewal options could not be agreed upon based on pricing considerations per City policy. The City emphasizes flexibility in scheduling, fast turnaround times for scheduling and rescheduling, and flexible onsite services as key expectations for the new contract.

RE: ADDENDUM 1
11038-0-2022-AH
Drug and Alcohol Testing Services
Notice to All Bidders:
Please note the following responses to the questions received:
Question 1: I didn’t see Form D: Cost Proposal attached, can we get a copy?
Answer 1: Please see page 3 and 4 below for cost proposal.
Question 2. Who is your current TPA provider?
Answer 2: Comprehensive Drug Testing, Inc
Question 3. What laboratory do you currently use?
Answer 3: We do not select the lab. The lab is either included by the vendor or subcontracted by the vendor.
Question 4. What is your current pricing?
Answer 4: See attached pricing list, page 5.
Question 5. The RFP paperwork does not list the correct email address for Erin Hillson.
Answer 5: The email address for Erin Hillson should be EHillson@cityofmadison.com
Question 6: Who is your current TPA/provider(s) managing this drug testing program & SAMHSA LAB conducting the urine testing?
Answer 6: Comprehensive Drug Testing, Inc. is the TPA. We do not select the lab. The lab is either included by the vendor or subcontracted by the vendor.
Question 7: What current walk-in clinics do you use, and what are their hours? Are the current hours satisfactory to the City’s needs?
Answer 7: The current subcontractor is Drug and Alcohol Testing, 24/7, LLC. Current walk in hours include 8a-4:30p Monday through Thursday, and 8a-3:30p on Friday. We need collections at all hours of the day and for a variety of reasons, both on site and walk in hours. The primary need is onsite collections.
Question 8: How / Where might we obtain the current contract / pricing for these services being requested, or you could provide the current price sheet from the current contract?

Question 9: How often has “onsite” (at Transit facilities) events occurred, and are these scheduled or post accident situations? How many donors are present for scheduled onsites?
Answer 9: For FMCSA and FTA testing, these are scheduled ahead of time to be at the departments. If changes are needed, employees have to go to the walk in clinic then or go to a different onsite location. All random and follow up tests are done onsite. Post accident, pre-employment (except BA test may be onsite), and return to duty tests are either on site or at the walk in clinic, Drug and Alcohol Testing 24/7 LLC. The number of each of these tests varies. Numbers of donors present varies, but is often just 1 test.
Question 10: Will those in the “safety sensitive” position require the same category of drugs (a lookalike panel) of which is in the DOT panel, to mirror testing for all employees?
Answer 10: This question is unclear. All testing must be done in accordance with FTA and FMCSA standards.
Question 11: Where can we identify additional pricing that MAY be needed during the cost of the contract, that is not indicated on the price sheet. I do see one row for “Other”, but if there is more than 1 item, where would you like us to indicate such?
Answer 11: Please indicate in your pricing document all additional costs including but not limited to, testing costs, travel costs, cancellation costs, observed collection costs, after hours costs, etc.
Question 12: Why is this bid being solicited? Has all renewal option expired, or has there been an interruption with the service?
Answer 12: The original contract is complete and extensions could not be agreed upon based on pricing, per City policy.
Question 13: If given the ability, what would the City change moving forward with the next Contract, to raise the service level even higher than it is now? In other words, do you have a “wish list” of any additional services/features/expectations moving forward?
Answer 13: Expectations include flexibility in scheduling, fast turn-around times related to scheduling and rescheduling, and flexible onsite services.
Question 14: Being this RFP is soliciting DOT testing services, would the City be interested in price quotes on other required DOT services, like: DOT Physicals, Background checks, or MVRs? Or are these services on other contracts?
Answer 14: These services are being completed through a variety of other means outside of contracts.
Question 15: Would the City have interest in utilizing the ELECTRONIC CCF AND LONG FORM for collection needs, to reduce collector errors on paper CCFs, or the need to continuously order more when supplies run low?
Answer 15: Potentially.
Bidders must acknowledge receipt of this addendum accordingly on RFP Form B, Receipt Forms and Submittal Checklist.
