About this file

This is an Addendum to a state and local contract opportunity for pharmaceutical services at a healthcare facility in West Virginia. The facility operates with a current census of 78 residents and a maximum occupancy of 120, with resident lengths of stay ranging from several months to several years. The scope of services includes pharmacist oversight of medication management across one centralized medication room, one medication cart, and one emergency bag checked weekly for compliance. The facility utilizes an EHR system and will provide the pharmacist with electronic access to patient records. Medications are distributed through the Hershey Woody Williams VAMC in Huntington, WV, with delivery occurring via facility driver pickup of a lockbox. The contract requires base annual service hours estimated at 96 hours, with potential for additional hours due to emergencies or educational needs. In-service education sessions are estimated at 1-2 annually, and typical monthly controlled substance destruction volume is 10 units.

The contract requires a monthly report of services delivered and a timesheet to accompany invoicing. The facility experiences minimal 24-hour response inquiries, with typical monthly volume between 0-0.5 occurrences. The previous vendor's budget information cannot be disclosed per WV State Code 5A-3-11a, which prohibits disclosure of maximum budgeted amounts to vendors prior to contract award. The actual hours utilized in the most recent contract term may vary depending on vendor performance and agency needs, though 96 annual hours remains the reasonable base estimate. No specific contract term dates, renewal options, or pricing structures are detailed in this addendum.

1. What is the current census, projected census, licensed beds, and average length of stay?

We currently have 78 residents with a maximum occupancy of 120. Our length of stay varies from a few months to several years.

2. Does the facility use an EHR/eMAR, and will the pharmacist have electronic access or only have access to paper charts?

We do utilize an EHR system and will permit the pharmacist to have access to this system.

3. How many medication storage locations are in the scope of services (med rooms, carts, emergency kits), and what inspection frequency is expected for each?

We have one medication room where all medications are stored. There is a medication cart as well, however it is not pushed through the hallways/facility. We have one emergency bag that is kept in the medication room and checked for compliance once a week.

4. What is the facility’s current medication distribution model (vendor, delivery frequency, emergency/after-hours process)?

We utilize the Hershey Woody Williams VAMC in Huntington, WV for medication distribution. Our driver that drives our residents to their appointments picks up a lockbox of medications and delivers it to our nursing department.

5. What was the previous vendor’s budget, and what is the current budget for this contract period?

It is unknown what the previous vendor’s budget was nor would it be allowable to release that information according to WV State Code. §5A-3-11a. Negotiation when all bids exceed budget in requisition.

(a) Spending units shall include the maximum budgeted amount available for each purchase in a requisition submitted to the Purchasing Division. No person may disclose this maximum budgeted amount to any vendor prior to the award of a contract

6. What is the typical monthly volume of controlled substance destructions?

The typical monthly volume of controlled substances to be destroyed is 10.

7. The RFP estimates 96 annual hours—what were the actual hours utilized in the most recent contract term?

Actual hours would vary depending on the vendor and agency needs. 96 annual hours is a good estimate of base hours needed. More hours may be required in the event of an emergency or added education hours.

8. For the 24-hour response requirement, what is the typical monthly volume of inquiries?

Typical monthly volume would be 0-.5. It is an uncommon occurrence.

9. How many in-service education sessions are typically requested annually?

This could vary but estimated at 1-2 annually.

10. Are there any required deliverables/templates for documentation (e.g., monthly reports, intervention logs) to accompany invoicing?

A monthly report of services delivered as well as a timesheet must accompany invoicing.
