About this file

This document includes a past performance questionnaire and federal contract opportunity notice. The past performance questionnaire is a template for references to provide feedback on a contractor's past work. It requests ratings on quality, schedule, cost control, business relations, and personnel management. The federal contract opportunity is a special notice for direct care, supervision, and facilities maintenance services at an influx care facility in Greensboro, North Carolina. It describes the facility and states the agency's intent to issue a short-term sole source contract through May 2023, followed by a competitive solicitation in early 2024. Interested vendors are requested to provide feedback on the draft performance work statement and capabilities information.
