About this file
Summary
This is an Acceptance of Offer Form from the City of Phoenix, Arizona, Municipal Corporation, used to formalize contract awards for solicited goods or services. The form serves as the binding agreement between the City and contractors whose offers have been accepted following a competitive procurement process. The document establishes that contractors are bound to provide the materials or services specified in the attached contract, solicitation documents, amendments, and their submitted offer. Contractors are explicitly cautioned against commencing any billable work or providing materials or services until they receive formal purchase order or contract documentation from the City. The contract is executed by the City Manager and approved by the appropriate department director, with attestation by the City Clerk. The form includes a notation that it was approved as to form by the City Attorney on January 19, 2017, and does not require resubmission for legal review unless the document is materially altered.
For the specific Lifeguard Uniforms procurement, the City of Phoenix Parks and Recreation Department issued an Invitation for Bid for a five-year contract commencing April 1, 2026, covering 43 line items including women's training suits (Speedo Endurance+), men's swimwear (Speedo Marina Volley and O'Neill Santa Cruz styles), and men's and women's cover and crew shorts (Watermen brand). Pricing must be submitted on a per-unit, firm and fixed basis for the initial one-year period with City-approved annual price adjustments thereafter. Offerors must have been in operation for a minimum of five years, and all products must be new with no manufacturing defects, warranted for 12 months or the manufacturer's warranty, whichever is greater. The pre-offer conference was scheduled for February 2, 2026, written inquiries were due February 4, 2026, and sealed offers were due February 13, 2026. Delivery is specified at 30 days after receipt of order on an FOB destination basis, with payment within 30 to 45 calendar days of invoice receipt. Contractors must comply with federal civil rights requirements, DBE regulations, E-Verify worker verification, and obtain appropriate licenses and insurance coverage.
