About this file
This Acceptance of Offer Form is issued by the City of Phoenix, a municipal corporation in Arizona, to formalize the acceptance of a contractor's offer for lifeguard uniforms. The City of Phoenix Parks and Recreation Department seeks sealed offers for a five-year contract commencing April 1, 2026, covering 43 line items of lifeguard apparel including women's training suits (Speedo Endurance+), men's swimwear (Speedo Marina Volley and O'Neill Santa Cruz styles), and cover shorts and crew shorts in various sizes. A pre-offer conference is scheduled for February 2, 2026, at 10:00 a.m. via Microsoft Teams, with written inquiries due by February 4, 2026, at 2:00 p.m. Sealed offers must be submitted by February 13, 2026, at 2:00 p.m. through the City's Procurement Portal. Upon acceptance of an offer, the contractor becomes bound to the contract terms and is instructed not to commence billable work until receiving purchase order or contract documentation.
Pricing must be submitted on a per-unit basis with firm and fixed pricing for the initial one-year period, with City-approved price adjustments permitted annually thereafter if supported by documentation. All products must be new with no manufacturing defects, and the selected contractor must maintain a 12-month warranty or the manufacturer's warranty, whichever is greater. Delivery is specified at 30 days after receipt of order to various Parks and Recreation warehouses on an FOB destination basis, with payment within 30 to 45 calendar days of invoice receipt following final acceptance. Offerors must have been in operation for a minimum of five years and demonstrate experience providing lifeguard uniforms. Contractors must comply with Title VI of the Civil Rights Act, Americans with Disabilities Act, Section 504 of the Rehabilitation Act, DBE requirements under 49 CFR Part 26, Immigration Reform and Control Act compliance through E-Verify, and obtain required licenses and certificates of insurance. No substitutions of specified brands and product specifications are permitted without prior approval, and the City reserves the right to add, change, or delete quantities and items as circumstances require.
