This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, "Streamlined Procedures for Evaluation and Solicitation for Commercial Items," FAR 13.303 "Blanket Purchase Agreements (BPAs)" as supplemented with additional information included in this notice. 1.0. Description of Services. The work covered by this contract and its respective task/call orders against this contract shall consist of execution, performance, and management of Diving Operations including but not limited to underwater inspections, documentation, survey, filming, repair, maintenance, and ROV operations.
The contractor shall satisfactorily perform Diving Operations at U.S. Government property, structures, and facilities and/or U.S. Government Sponsored Diving Operations at structures, access areas, drainage features, creeks, wetlands, watersheds, shorelines, docks, loading ramps, boat ramps, swimming areas, piers, navigational locks, lock chambers, dams, spillways and other underwater work in accordance with the contract specifications contained herein and as specified in task/call orders issued against this contract. All work shall be performed to the satisfaction of the Contracting Officer.
1.0.1. Contract Areas. Contract areas consist of but are not limited to underwater work at alluvial floodplains, federal owned property, lock and dams, facilities, recreational sites, shorelines, spillways, environmental management areas, watersheds located on or along the Mississippi River from Mississippi River Mile 170.0 to 284.0 and on or along the Missouri River within the Missouri Counties of St. Louis, Franklin, St. Charles and on or along the Illinois River from Illinois River Mile 0.0 to 80.0 within the Illinois Counties of Jersey, Calhoun, Greene, Pike, Scott, Morgan, and Cass. Contract areas also include the following Primary locations for diving operations including the Physical Support and Service Base, St. Louis, MO; Lock and Dam 24, Clarksville, MO; Lock and Dam 25, Winfield, MO; Melvin Price Lock and Dam, Alton, IL; Lock and Dam 27, Granite City, IL; Kaskaskia Lock and Dam, Modoc, IL; Mark Twain Lake, Monroe
City, MO; Lake Shelbyville, Shelbyville, IL; Carlyle Lake, Carlyle, IL; Rend Lake, Benton, IL;
Wappapello Lake, Wappapello, MO. Primary locations are shown by map in Appendix B.
Specific contract areas will be specified within each task/call order.
1.0.2. Contract Period. The period of performance for this contract will be for one year commencing from the date of award with four option years, if exercised.
1.0.3. Normal Working Hours. Work shall typically be scheduled to be performed during daylight hours Monday through Friday, 7:00 a.m. – 5:00 p.m. Situations may arise that require diving operations to be performed outside this typical schedule including diving operations to be performed Sunday through Saturday at all hours, including holidays.
1.1. Method and Conditions of Diving. Contractor shall ONLY use the Surface Supply Air (SSA) method of diving for all portions of this diving contract. Dive team manning levels, referenced later in this specification, are based on SSA diving. The Contractor shall immediately notify, inform and document in writing and/or photographs if requested, to the Contracting Officer of any unusual conditions observed.
1.2. Four (4) Man Dive Team with Floating Plant. A four (4) man dive team shall consist of a minimum of four (4) personnel with Floating Plant and supplement equipment. Personnel furnished shall consist of a minimum of one (1) Diving Supervisor, one (1) Diver, one (1) Standby Diver, and one (1) Tender.
Personnel shall be capable of operating and/or utilizing all equipment and floating plant as specified within the contract specifications. Floating Plant shall be furnished as part of this line item and shall consist of one boat or work flat capable of supporting up to a minimum of six (6) personnel and equipment required to perform diving operations and be capable of navigating to, and in around designated locations.
The following additional equipment shall be furnished for use with the floating plant at all times and such rental and use of the following equipment shall be considered incidental to cost associated with, “Four (4) Man Dive Team with Floating Plant”:
Two-way communication systems between the divers, standby divers, and dive supervisors.
Air compressors and ancillary equipment Harnesses, helmets, hoses, fuel Ladders, lanyards, and ropes Diving Equipment and Gear required to perform Dives Miscellaneous Small Tools to include but not limited to knives, wrenches, hammers, saws, drills, and small tools associated with the divers trade.
Underwater photo equipment Underwater video equipment Small pneumatic tools, hoses & related equipment.
Hydraulic Power unit capable of supplying a minimum 2000 psi with a minimum of 150 linear feet of supply hose.
Global positioning system (GPS) unit Miscellaneous Floating Plant Equipment required for operation and maintenance of the Floating
Plant.
Underwater welding rig. Underwater welding rig shall include all required fuels, seals, regulators, hoses, valves, and torches.
Underwater burning rig. Underwater burning rig shall include all required fuels, seals, regulators, hoses, valves and torches.
Underwater hydraulic grinder. Underwater hydraulic grinder shall include all couplers, hose whips, seals, fittings, grinding wheels, guards and brushes.
Underwater hydraulic impact wrench. Underwater hydraulic impact wrench shall include all seals, bits, valves, couplers, hose whips, guards and attachments.
Underwater hydraulic drill. Underwater hydraulic impact drill shall include all seals, bits, valves, couplers, hose whips, guards and attachments.
Light Plant for operations of diving during night operations.
1.2.1. 8 Hour Operation. The Contracting Officer will compute measurement for “Four Man Dive Team with Floating Plant - 8 Hour Operation” in days of productive operation or performance consisting of 1~8 Hour shift per day for all equipment and personnel required per the requirements for “Four (4) Man Dive Team with Floating Plant”. In computing the number of days of productive operation, only the days of actual operation or performance of work will be considered. Standby or idle time shall not be paid. Days of actual operation shall include also site-to-site mobilizations within each contract work area and time spent receiving field instructions from the Contracting Officer or Contracting Officer’s Representative. Measurement for payment shall be rounded up to the nearest whole Day of Productive operation.
1.2.2. 12 Hour Operation. The Contracting Officer will compute measurement for “Four Man Dive Team with Floating Plant - 12 Hour Operation” in days of productive operation or performance consisting of 1~12 Hour shift per day for all equipment and personnel required per the requirements for “Four (4) Man Dive Team with Floating Plant”. In computing the number of days of productive operation, only the days of actual operation or performance of work will be considered. Standby or idle time shall not be paid. Days of actual operation shall include also site-to-site mobilizations within each contract work area and time spent receiving field
1.2.3. 24 Hour Operation. The Contracting Officer will compute measurement for “Four Man Dive
Team with Floating Plant - 24 Hour Operation” in days of productive operation or performance consisting of 1~24 Hour shift per day for all equipment and personnel required per the requirements for “Four (4) Man Dive Team with Floating Plant”. In computing the number of days of productive operation, only the days of actual operation or performance of work will be considered. Standby or idle time shall not be paid. Days of actual operation shall include also site-to-site mobilizations within each contract work area and time spent receiving field instructions from the Contracting Officer or Contracting Officer’s Representative. Measurement for payment shall be rounded up to the nearest whole Day of Productive operation.

1.3.3. 24 Hour Operation. The Contracting Officer will compute measurement for “Five Man Dive
Team with Floating Plant - 24 Hour Operation” in days of productive operation or performance consisting of 1~24 Hour shift per day for all equipment and personnel required per the requirements for “Five (5) Man Dive Team with Floating Plant”. In computing the number of days of productive operation, only the days of actual operation or performance of work will be considered. Standby or idle time shall not be paid. Days of actual operation shall include also site-to-site mobilizations within each contract work area and time spent receiving field instructions from the Contracting Officer or Contracting Officer’s Representative. Measurement for payment shall be rounded up to the nearest whole Day of Productive operation.
1.4. Additional Diver/Tender. Additional divers and tenders shall be thoroughly qualified in their trade and in the use of equipment as specified within the contract specifications in order that maximum capacity may be obtained. An additional diver shall be accompanied by an additional tender at all times during diving operations. All personnel furnished by the Contractor shall be subject to the approval of the Contracting Officer.
1.5. Standby - Four (4) Man Dive Team with Floating Plant. A standby - four (4) man dive team with floating plant shall consist of all equipment and personnel required per the requirements for “Four (4) Man Dive Team with Floating Plant”. In computing the number of days of standby operation, only the hours of actual standby operation while the contractor is actively on the jobsite will be considered.
Hours of standby operation shall include also site-to-site mobilizations within each contract work area and time spent receiving field instructions from the Contracting Officer or Contracting Officer’s Representative. Measurement for payment shall be rounded up to the nearest whole hour of standby operation.
1.6. Standby - Five (5) Man Dive Team with Floating Plant. A standby - five (5) man dive team with floating plant shall consist of all equipment and personnel required per the requirements for “Five (5) Man Dive Team with Floating Plant”. In computing the number of days of standby operation, only the hours of actual standby operation while the contractor is actively on the jobsite will be considered.
Hours of standby operation shall include also site-to-site mobilizations within each contract work area and time spent receiving field instructions from the Contracting Officer or Contracting Officer’s Representative. Measurement for payment shall be rounded up to the nearest whole hour of standby operation.
1.7. Miscellaneous Equipment and Materials. The equipment to be furnished shall be at a minimum, but not limited to, current shields including required equipment and plant for utilization, additional work flats, remotely operated vehicles (ROV’s), and Specialty underwater dive equipment and materials.
Miscellaneous Equipment and Materials shall be utilized for equipment and materials not specified within the contract specifications to be utilized or measured and paid for as part of another line item.
Miscellaneous equipment and materials shall be specified within each task/call order.
2.0. Service Delivery Summary.
2.0.1. Service Delivery Summary (SDS). The contract service requirements are summarized in performance objectives that relate directly to specific job requirements. The maximum number of defective items in a lot that can be considered satisfactory is called the performance threshold.
The performance threshold for each service to be performed can be found in the chart below.
When the number of defects for a specified service (or lot) is below the performance threshold, the work will be considered satisfactory. However, the Contractor is still required to correct the deficiencies if possible. Each service that is performed will be determined acceptable or deficient according to the inspection rating that is received. The SDS and the contractor’s Quality Control Plan provide information on contract requirements, the expected level of contractor performance and the expected method of government validation and confirmation of services provided.

2.1. Quality Control Plan. Prior to commencement of work the Contractor shall submit his Quality Control
Plan. The Plan shall identify personnel, procedures, instructions, records and forms to be used. The Plan shall include a description of the quality control organization, name, duties, responsibilities and authorities of each person assigned a quality control function and a copy of the letter to the quality control manager signed by an authorized official of the firm which describes the responsibilities and delegates the authorities of the quality control manager.
2.2. Quality Assurance. The government shall inspect and evaluate the contractor’s performance to ensure services are received in accordance with the requirements set forth in this contract. The KO or COR shall inspect by validating actual work performance, physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, conferring with facility managers, or otherwise inspecting the task or its results to determine whether or not performance meets the standards contained in this PWS.
2.3. Performance Assessment.
2.3.1. Periodic Validations. This method requires the COR to employ a “spot check” style of evaluation. Periodic validations will be conducted on a scheduled basis (daily, weekly, monthly, quarterly, semi-annual or annually) and may be adjusted, based on quality trends. The number of times that a service is provided by the Contractor for a given bid item during a specific time period, (day, week, etc.) is called a “lot”. Any unsatisfactory inspection (defect) result shall be recorded, and the contractor shall re-perform the service after notification by the CO or COR.
Failing to meet the performance threshold as outlined in the SDS for any of these performance objectives in any one month period shall result in a warning or letter of concern from the CO.
Failing to meet the performance threshold as outlined in the SDS of these performance objectives in any combination for any two or more consecutive or non-consecutive months during a contract period shall constitute a deduction.
3.0. Government-Furnished Equipment, Materials, and Supplies.
3.1. General. The Government will supply to the Contractor, for use only in connection with this contract, miscellaneous property and materials not listed within the specifications at times necessary for the use by the Contractor to provide necessary work as directed by the Contracting Officer or Contracting Officer Authorized Representative.
3.2. Keys. The Government will furnish the necessary keys for access to facilities. The Contractor shall evidence receipt of such materials by signing the project’s key control sheet. NOTE: DUPLICATION OF GOVERNMENT-FURNISHED KEYS IS NOT PERMITTED. The Contractor shall immediately report the loss of keys to the Contracting Officer. In the event that a Government-furnished key is lost, the Contractor is responsible for reimbursement of all costs incurred by the Government for replacement of entire lock systems throughout the lake project, which are affected by the lost key. Contractor costs shall include, but not be limited to, Government labor, replacement lock costs, replacement key costs, tumbler reworking and all contracted labor costs required to replace lock systems. All lock systems required to be replaced will be equal quality to lock systems presently in place. The Government will determine the specific replacement lock system.
4.0. Contractor-Furnished Equipment, Materials, and Supplies.
4.1. General. All contractor-furnished equipment and materials used in the performance of this contract shall be of suitable size, type and quality for the intended use. The contractor shall furnish all floating plant, vessels, plant, labor, equipment, dive equipment, communication systems, underwater tools, specialty tools, operators, supervision/site superintendent, supplies, and materials, and incidentals necessary to satisfactorily perform Diving Operations at U.S. Government property and/or U.S. Government Sponsored Diving Operations. Contractor-furnished equipment shall be inspected daily by the contractor and maintained in a safe and properly working condition. The Contracting Officer reserves the right to inspect any and all equipment, materials, and supplies prior to award of this contract and during the course of this contract.
4.2. Property Storage. The Contractor may store Contractor equipment and supplies/materials at the work site during continuous working days as approved in advance by the Contracting Officer. Contractor equipment shall be removed from the work site within two (2) days after the completion of the work described within the task/call order unless prior approval is given by the Contracting Officer. The Government assumes no responsibility for the safety or security of Contractor equipment and supplies/materials left on Government property.
4.3. Vehicles and Vessels. The Contractor shall keep all vehicles and vessels clean and in a safe operating condition and shall be clearly marked with identification showing, Contractor's name and type of business.
Vehicle and vessel signs shall be of professional quality and shall be approved by the Contracting Officer.
4.4. Communications. The Contractor shall provide crew leaders with a reliable and functioning communication system with call signs/numbers furnished to the Government. The system provided shall insure communications between the Government and the Contractor crew leader(s) during all hours of scheduled work.
4.5 Sustainability. In accordance with (IAW) the U.S. Army Corps of Engineer (USACE) Acquisition Instruction (UAI), USACE shall advance sustainable acquisition and the Army Green Procurement Guide by ensuring that all applicable new contract actions include relevant, federally mandated, sustainability requirements.
4.5.1. EPA Designated. Under the Comprehensive Procurement Guidelines (CPG) program the EPA designates products that are, or can be made with recovered materials, and recommends practices for buying these products. Any designated product that is being offered or supplied under this contract shall meet the minimum recommended content levels as identified under the CPG program. The contractor is encouraged to visit http://www.epa.gov/epawaste /conserve/tools/cpg/products for a complete list of designated products and the associated recommended content levels. Offerors must be able to demonstrate that each offered product meets minimum content levels upon request.
4.5.2. Bio-Based. The United States Department of Agriculture (USDA) designates certain bio-based products for federal procurement and specified minimum bio-based content levels for those products. Any designated product that is being offered or supplied under this contract shall meet USDA BioPreferred’s minimum bio-based content level. Visit the BioPreferred website (https://www.biopreferred.gov/BioPreferred/) for the complete list of designated products and the associate minimum bio-based content level requirements.
5.0. General Information.
5.1. Scope of Work. Work shall consist of execution, performance, and management of Diving Operations including but not limited to underwater inspections, documentation, survey, filming, repair, and maintenance. The contractor shall furnish all floating plant, vessels, plant, labor, equipment, dive equipment, communication systems, underwater tools, specialty tools, operators, supervision/site superintendent, supplies, and materials, and incidentals necessary to satisfactorily perform Diving Operations at U.S. Government property, structures, and facilities and/or U.S. Government Sponsored Diving Operations at structures, access areas, drainage features, creeks, wetlands, watersheds, shorelines, docks, loading ramps, boat ramps, swimming areas, piers, navigational locks, lock chambers, dams, spillways and other underwater work in accordance with the contract specifications contained herein and as specified in task/call orders issued against this contract. All work shall be performed to the satisfaction of the Contracting Officer.
5.2. Safety. The contractor shall comply with EM 385-1-1 (e.g., contracts that include the Accident Prevention clause at FAR 52.235-13 and/or other safety provisions). EM 385-1-1 and its changes are available at http://publications.usace.army.mil/publications/eng-manuals/ (at the HQ homepage, select Safety and Occupational Health). The Contractor shall be responsible for complying with the current edition and all changes posted on the web as of the effective date of this solicitation. The contactor shall also comply with the most recently published version of OSHA 29 CFR 1910 Subpart T, Commercial Diving Operations. The contractor The Government may inspect all equipment and materials. Any equipment or material found not in conformity with the requirements of this contract shall be removed from Government property immediately. The Contractor shall develop and submit an Accident Prevention Program using the forms located in Appendices C-1, C-2, C-3, and C-4 together with a copy of the Company Safety Policy.
5.2.1. Safety Exposure Report. The Contractor shall submit, in writing, a monthly safety exposure report for any month that work was performed under this contract. The report shall be submitted by the fifth (5th) working day of the following month, (i.e. January’s report is due by the fifth working day of February). This report shall include:
5.2.1.1. The total employee hours worked by employees during the preceding month, and;
5.2.1.2. Detailed descriptions of any accidents or injuries sustained while working on this contract by their employees during the preceding month, including copies of documents for claims under Workers Compensation.
5.2.2. Safety Indoctrination. The Contractor shall be responsible for initiating and maintaining a safety and health program which complies with the safety and health references described above.
The program shall include initial indoctrination and continuing training for all employees who shall work under this contract.
5.2.3. Required Safety Equipment for Non-Divers. Except for the diver in the water and the stand-by diver, all personnel on the dive team shall wear safety equipment (flotation aids, hard hats, hearing/eye protection, etc.) where required by EM 385-1-1. The stand-by diver is exempt from the requirements, but is required to wear a hard had when in a construction zone. When no divers are in the water, all dive team personnel shall wear the required safety equipment without exception where required by EM 385-1-1.
5.2.4. Site Safety and Health Officer (SSHO). Site Safety and Health Officer (SSHO) shall be provided at the work site at all times to perform safety and occupational health management, surveillance, inspections and safety enforcement for the Contractor. The Contractor Quality Control (QC) person can be the SSHO on this project. The SSHO shall meet the following requirements.
Work on similar projects.
10-hour OSHA safety class or equivalent within the last three (3) years.
Competent person training as needed.
5.3. Submittals.
5.3.1. General. Contractor Dive Submittals are required for all diving operations. At a minimum the submittals listed in this section are required, however additional submittals may be required for individual task/call orders as described in EM 385-1-1 Appendix O. As certifications expire, contractor will ensure that current certifications for personnel and equipment are submitted to the
DDC.
5.3.2. Pre-Construction Submittals. The following submittals are required within 14 calendar days of contract award. These submittals must be updated on a regular basis throughout the duration of the contract an as required for individual diving operations. These submittals must be approved by the Government prior to the start of work under this contract.
5.3.3. Contractor Work Plan. Prior to the Post Award Orientation and prior to diving operations, the Contractor shall submit to the Contracting Officer or Authorized Representative thereof for review and approval, the Contractor Work Plan in accordance with EM 385-1-1. Any Contractor requested changes to an approved Contractor Work Plan shall be submitted to the Contracting Officer for review and approval no later than twenty-four (24) hours prior to implementation of any change. The Contractor Work Plan shall include at a minimum, but is not limited to, the following;
The names(s), mailing address(es) and telephone number(s) of the Contractor and/or persons of authority through which they can be contacted during regular and irregular business hours
The name(s) and telephone number(s) of the Contractor Representative(s) to whom deficiencies are to be reported and through which they can be contacted during regular business hours, The number of personnel and how personnel are to be used in the performance of the work, The methods to be used in one performance of each phase in area of work, The equipment and materials to be used in performance of the work, The Contractor's Accident Prevention Program, as required by EM 385-1-1
5.3.4. Safe Practices Manual. Contractor’s shall develop, submit, and maintain a safe practices manual that encompasses the contractor’s entire diving program and shall be available at all times to the Government representative and all dive team members at each diving location. When Contractor’s dive program changes, it is the responsibility of the contractor to maintain an up-to-date Safe Practices Manual on file with the DDC or Contracting Officer’s Representative. The Safe Practices Manual shall include, as a minimum, the following:
Safety procedures and checklists;
Assignments and responsibilities of dive team members in general terms;
Equipment certification, procedures, and checklists; it is the contractor’s responsibility to ensure current equipment certifications are on file with the DDC (see EM 385-1-1, Section 30.F for testing requirements).
Emergency procedures for fire, equipment failure, adverse weather conditions, and medical illness or injury and;
Requirements for inspections.
5.3.5. Dive Personnel Qualifications. Dive Personnel Qualifications shall include at a minimum the following:
5.3.5.1. Diver Physical Examinations. The Contractor shall insure that all divers are medically fit to dive. This shall include a physical examination within the previous 12-months, or following any physical injury which may affect the diver’s fitness if such an injury has occurred within the past 12-months. In addition, the Dive Supervisor shall verify the diver’s health and well being at the dive site before allowing the diver to enter the water.
A copy of each diver’s medical clearance certificate, signed by a licensed physician knowledgeable in the physiology of diving, shall be kept on file with the DDC. The clearance shall indicate that the diver is physically qualified to perform diving work, and detailing any limitations the individual may have. The physicals shall have been performed within 365-days of the start of the diving operations. The Contractor shall ensure that each diver’s physical remains current throughout the contract period, and shall furnish copies of any subsequent diver physicals to the DDC.
5.3.6. Copies of the following certifications shall be kept on file with the DDC for all dive team members. It is the responsibility of the Contractor to maintain dive team certifications and provide updated copies to the DDC throughout the life of the contract. All members of the dive team shall have current certifications for each of the following rescue methods from a nationally recognized organization.

5.3.7. Divers shall be experienced at the depth they will be diving, the task to be performed, and the equipment to be used. Contractor shall provide dive-log evidence that each dive team member has training and experience consistent with the performance requirements of the scope of work.
Qualifications shall consist of certification of successful completion of training as a commercial working diver from an accredited commercial dive school. An ADCI card or similar certification from an internationally recognized commercial diving organization may be substituted as proof of training for divers demonstrating more than 5 years of diving experience within the six years preceding commencement of dive operations. Copies of the qualifications for each diver shall be provided by the Contractor and kept on file with the DDC.
5.3.8. Divers performing underwater structure inspections shall be trained and certified by a recognized licensing organization. Copies of training certificates shall be kept on file with the DDC.
5.3.9. Environmental Protection Plan. As further detailed in 5.11. Environmental Protection, an Environmental Protection Plan (EPP) shall be submitted to the Government within 14 calendar days of award of this contract and updated as needed through the life of the contract.
5.4. Pre-Dive Submittals. The following submittals are required at least 5 calendar days after the date of receipt of a written task/call order or notification to proceed by the Contracting Officer and prior to the start of each separate diving operation associated with this contract and shall be submitted to the DDC or Contracting Officer’s Representative. In the event of emergency dive operations, these documents must be submitted at least 8 hours after the date of receipt of a written task/call order or notification to proceed by the Contracting Officer and prior to emergency diving operations. Each of these documents will become part of the project file. Penetration diving, contaminated environment diving, and in areas where differential pressure entrapment hazards exist, will be specifically addressed in each document when they are anticipated as part of a diving operation.
5.4.1. Contractor Representative(s). Contractor shall provide in writing to the Contracting Officer, the name(s) of the Contractor Representative(s) who will be on site with the authority to conduct overall management, coordination, and supervision whenever work specified herein is being performed. The Contractor Representative(s) will serve as the central point(s) of contact with the Government for performance of all work under this contract. The Contractor Representative(s) shall coordinate directly with the Government for normal day-to-day administration of this contract and shall be present at post award orientation.
5.4.2. Dive Operation Plan(s). As a minimum the plan will contain the following:
Detailed description of the mission;
Names and specific duties of dive team members;
Date, time, and location of operation;
Diving mode utilized (SCUBA, Surface Supplied Air), including description of the backup air supply, as required;
Nature of work to be performed by divers;
Surface and underwater conditions, to include visibility, temperature, currents, etc.
Thermal protection will be required when appropriate;
Maximum depth and bottom time, and;
Special tools as required.
The dive plan shall also include the following statement: “If for any reason the dive plan is altered in mission, depth, personnel, or equipment, the DDC will be contacted prior to review and accept the alterations prior to the actual operation.”
5.4.3. Activity Hazard Analyses. An AHA represents the dive team’s best effort to anticipate and mitigate or prevent the adverse effects of equipment failure, extreme weather/environmental conditions, or other hazardous/unexpected situations. Each AHA shall be job specific and address each phase of work to include the hazards associated with flying after diving. Lock out/tag out procedures, and procedures dealing with differential pressures will be included where appropriate. Some dives may be sufficiently complex to warrant several separate AHAs. The AHA will be covered in detail at the pre-dive conference. If safe clearance procedures are required for the diving operation, the diving supervisor will walk through the clearances to assure they are in place and redundant where possible prior to the commencement of the diving operation.
5.4.4. Emergency Management Plan. An emergency management plan will be prepared for each dive and approved by the DDC. The minimum content of the plan shall be as follows:
Location and phone number(s) of nearest operational decompression chamber (if not located at the dive site);
Location and phone number(s) of the nearest hospital(s);
Location and phone number(s) of the nearest US Coast Guard Rescue Coordination Center;
Description of an emergency victim transport plan including phone number(s) of appropriate emergency transport services;
Procedures and phone numbers or other lines of communications to activate emergency services at the facility where the work is being performed;
Identify primary and secondary means of communication to activate the emergency management system Actions upon loss of vital support equipment;
Actions upon loss of gas supply;
Action upon loss of communication;
Lost diver plan;
Injured diver plan;
Actions upon discovery of fire;
Diver blow up/over rapid ascent to surface;
Diver loss of consciousness;
Injury/illness of member of surface crew with diver in water;
Procedure to deal with entrapped or fouled diver including fouled umbilical.
5.5. Post Dive Submittals.
5.5.1. Reports. At the end of each task/call order the Contractor shall submit a report documenting all actions of the dive and all documentation requested within the scope of work for the individual task/call order. The report format shall be coordinated with and accepted by the DDC prior to any work being done under this contract. The report shall include but not be limited to a description of the work performed; condition of all items surveyed or inspected, complications of the dive, suggested repairs/fixes and a copy of all dive logs. Clear photographs shall be included to supplement the text descriptions where applicable. The completed report shall be signed by an official designee of the Contractor and delivered to the Contracting Officer’s Representative within 10 calendar days of the completion of diving operations, and shall remain the property of the Government. The Contractor shall submit at least one electronic copy and one hard copy of each dive report to the Contracting Officer or Authorized Representative Thereof.
5.6. Applicable Publications. The Contractor shall perform all work in accordance with the following applicable publications, including but not limited to:
5.6.1. Manufacturer’s Recommendations. For equipment, supplies and materials used/or inspected by the Contractor.
5.6.2. Equipment Operating Manuals. For all equipment used by the Contractor during this contract duration.
5.6.3. Industrial Standards and Codes (latest editions). Includes the following but not limited to:

5.7. Commencement, Schedule, and Completion. The Contractor shall not commence any work prior to the Contracting Officer's issuance of a written task/call order unless specified otherwise by the Contracting Officer. Unless otherwise specified within an individual task/call order, the Contractor shall be required to commence work on individual task/call orders within seven (7) calendar days after the date of receipt of the written task/call order or notification to proceed by the Contracting Officer. The Contractor shall be required to commence work on individual task/call orders involving emergency diving operations within twenty-four (24) hours after the date of receipt of a written task/call order or notification to proceed by the Contracting Officer for emergency services. The Contractor shall coordinate with the Government any and all Contractor operations in and around other contractors, work crews, or government employees. In the event that weather conditions or flooding of contract areas will not permit performance of work the Contractor will suspend work until such time as the conditions improve and become suitable for performance of work. No adjustment in contract price will be made for any suspension of work. Under any condition, if work was not completed as scheduled and no advance notice was given to the Government and agreed to, then the Government may reject the services for payment and/or assess a re-inspection fee. A re-inspection fee may also be assessed if the Contractor was required to re-perform services due to deficiencies.
5.7.1. Operating Time. The equipment and labor shall be available for operation for eight (8) hours per day during daylight hours, Monday through Friday, weather and other conditions permitting. Overtime during diving operations may be required Monday through Friday for up to a total of 12 hours per employee per day. During emergencies as declared by the Contracting Officer or Contracting Officer’s Representative thereof, equipment and labor shall be available for up to 24 hour operations Sunday through Saturday. No work will be performed when river and/or weather conditions, in the opinion of the Contracting Officer or Contracting Officer’s Representative, at the site will not permit efficient and economical operations.
5.8. Suspension of Operations. Should river and/or weather conditions develop during the life of the task/call order, which will, in the opinion of the Contracting Officer, make further work impracticable or undesirable, the Contractor will be ordered to suspend operations until such time as the working conditions improve and become suitable for further work to proceed. The Contractor shall be required to recommence work within 48 hours as directed by the Contracting Officer or Contracting Officer’s Representative. If work is to be suspended for mobilized equipment and personnel the Contracting Officer will provide one of the following directions:
5.8.1. Remobilization. If it is in the best interest of the Government, all equipment will be directed to be demobilized until such time as working conditions improve and become suitable for further work to proceed. At such time the Contractor will be directed to remobilize and complete all remaining work. Payment for Additional Mobilizations and Demobilizations shall be at the Contract Line Item and Unit Price for Mobilization and Demobilization of equipment and personnel to which the work relates.
5.8.2. Standby. The suspension may be ordered to remain in effect until such time as working conditions improve and become suitable for further work to proceed. Payment for Standby will be made at the Contract Line Item and Unit Price for Standby of equipment and personnel to which the work relates. No separate payment or direct payment will be made for Contractor cost associated with Standby in excess of those cost provided for under paragraph 5.18., contained herein and all additional cost in connection therewith shall be included in other items for which payment is provided.
5.8.3. Termination. Task/call order times for work required within the specifications of this contract and for work directly associated with the performance of the specifications of this contract may be terminated in accordance with the Contract Clause entitled "Termination for Convenience of the Government".
5.9. Pre-Dive Coordination Meetings. Prior to the initial work on each dive operation, a Pre-Dive Conference shall be held with key personnel designated by the DDC to discuss the Dive Operations plan, AHA, and Emergency Plan and any modifications needed.
