Appendix D-1

Master Work Plan

The master work plan will include at a minimum the following:

1. The equipment and materials to be used

2. The name(s) and telephone number(s) of supervisor(s) to whom deficiencies should be reported.

3. The name(s) and telephone number(s) of supervisor(s)

4. The telephone number(s) and mailing address of the Contractor through which he can normally be contacted on a daily basis.

5. The telephone number(s) where persons of authority within the Contractor’s organization can be reached during non-duty hours.

6. The Contractor’s Quality Control Plan including:

a. Identification of authorized Quality Control Inspectors (QCI), whose duty shall be to verify contract compliance, their vehicles, and work/inspection schedule.

b. An inspection method of identifying deficiencies in the quality of services performed before the level of performance becomes unacceptable.

c. A method and response time for correcting minor deficiencies and unacceptable work that the Contractor is requested to re-perform.

d. A Daily Quality Control Report covering the bid items completed, which shall include, as a minimum, the form found in Appendix I.

(Note: The Quality Control Reports shall be maintained by the Contractor for the duration of the contract and will be collected by the Contracting Officer once per week.)

7. The Contractor’s Accident Prevention Program, as required by EM 385-1-1, including:

a. The Administrative Plan, see Appendix C, and

b. The Job Hazard Analysis, see Appendix C.
