SAFETY INSPECTION CHECK LIST FOR DIVING OPERATIONS

1. PERSONNEL CHECK
Check with District Diving Coordinator to verify that all medical/first-aid/CPR and DAN paper work is on file in the Safety Office.
Current medical examination for each diver signed by a licensed physician - must be within the last 12 months (30.A.10) Diving or tending certification. (30.A.08)
Current First Aid certification for all team members. (Renewed every 3 yrs.) (30.A.09)
Current CPR certification for all team members. (Renewed annually) (30.A.09)
2. ADMINISTRATIVE CHECK
Is the contractor Safe Practice Manual on site and reviewed at lease two of the following: DDC/ADC/DSR, prior to starting of diving operations? (30.A.14 and 30.A.16) Has Dive Operations Plan been developed, accepted and reviewed at lease two of the following: DDC/ADC/DSR, prior to starting of diving operations? (30.A.14 & 30.A.15) Has an Emergency Management Plan been developed for each dive and reviewed at lease two of the following:
DDC/ADC/DSR, prior to starting of diving operations? (30.A.14 & 30.A.19) Prior to initial work on each dive operation, has the Pre-Dive Conference with key personnel designated by the DDC to discuss the Dive Operation Plan, AHA, and Emergency Plan and any modifications needed? (30.A.20&21) If the dive has been altered with minor to moderate revisions to an accepted plan, has the DDC or ADC revised and accepted the plan? Note: Major revisions require two-person review including DDC or ADC acceptance (30.A.23) Have all valves, switches, controls, and equipment components been property locked out and/or tagged out in accordance with Section 12 of this manual? 3. EQUIPMENT CHECK
A. Air Compressor (30.F) Air compressor tagged and tested by an accredited testing laboratory every six months for:(30.F.05)
Carbon Monoxide <20 PPM (30.F.05(c)(1))
Carbon Dioxide <+1,000 PPM (30.F.05(c)(2))
Oil mist <5 milligrams per cubic meter (30.F.05(c)(3))
Air shall not contain a level of hydrocarbons greater than 25 PPM (30.F.05(c)(4))
Is air intake located away from exhaust? (30.F.04(4))
Does compressor have adequate capacity to deliver 4.5cfm actual to each diver? (30.F.03)
Each depth gauge shall be deadweight tested or calibrated against a master reference gauge every 6 months (29 CFR 1910.430(g)
Appendix F Dive Safety Inspection Checklist
B. Surface Supplied Air Operations (30.D) NOTE: Corps of Engineers General Safety Requirements (EM 385-1-1 (Sep 08) References as shown in parentheses. Yes No Are SSA dives limited to depths of less than 100 feet (? (30.D.03) NOTE: SSA Dives at depths exceeding 110 feet must be approved by the District Dive Coordinator and HQUSACE Dive Safety Program Manager. Additional controls are required.
Is each diver continuously tended with no more than one diver per tender regardless of depth (standby diver)?
(30.D.05(a)) Is the standby diver dressed out and readily available? Note: Only the headgear may be removed after it has been tested for proper operation. (30.B.03(a)) Are bailout bottles fully charged and worn? (30.F.07)
Does the reserve breathing air supply have in-line capable of supporting the dive operation including capacity to recover diver and complete emergency recompression (if required)? (30.D.05(d)) Have all lights, special tools been tested? (30.F.01) (30.F.13)

Are safety harness with positive buckling device and an attachment point for the umbilical and a lifting point worn?

C. Breathing air supply hoses (30.F.06) Have breathing hoses been tested prior to being placed in service, after any repair, modification or alteration and at least every 12 months thereafter to 1.5 times the working pressure. Have umbilical assemblies been tested at the same interval by subjecting each hose-to-fitting connection to a 200 pound axial load? (30.F.06(f)) Is the umbilical marked at 10ft. increments to 100ft. from the diver end? (30.F.06(c))
D. Physical examinations – Physicians/Licensed Healthcare Professionals (PLHCP) can sign physicals and health assessments (EM 385-1-1, Appendix Q, Item Q-5)
4. Scuba Diving Operations
SCUBA DIVING OPERATIONS SHALL NOT BE CONDUCTED WITHOUT THE WRITTEN APPROVAL OF
THE DISTRICT DIVING COORDINATOR
Notes:
The following information must be submitted to the Dive Coordinator after each dive is completed.
1) Dive Safety Meeting
2) Minutes of the dive, with a closing statement stating i.e. Job completed without an incidents or accident.
3) Copy of the inspection form MVN 385-40
Note: Differential pressure cannot exceed 6” for any lock or dam diving operations (Ref. EM 385-1-1, Paragraph 01.A.01).
